非侵袭性真菌性鼻窦炎鼻内镜手术及围手术期治疗

目的观察非侵袭性真菌性鼻窦炎鼻内镜手术治疗效果及围手术期治疗对疗效的影响。方法回顾性分析2008年6月-2010年12月诊治的86例非侵袭性真菌性鼻窦炎患者的临床特征,总结鼻内镜手术治疗效果及围手术期抗生素、黏液促排剂、鼻喷激素、鼻冲洗等综合治疗的作用。结果 86例患者均一次治愈,随访1～3年,无复发;围手术期综合治疗有利于减少术中出血,减轻术后黏膜水肿、减少分泌物,缩短病程。结论鼻内镜下彻底清除病灶是非侵袭性真菌性鼻窦炎安全、有效的一种治疗方法;合理的围手术期综合治疗具有其积极的作用。

Nasal Endoscopic Surgery for Non-invasive Fungal Sinusitis and Drug Therapy during the Perioperative Period

Objective To observe the therapeutic effect of nasal endoscopic surgery for non-invasive fungal sinusitis and the influence of drug therapy during the perioperative period on the effect.Methods Clinical characteristics of 86 patients with non-invasive fungal sinusitis who received diagnosis and treatment in our department between June 2008 and December 2010 were analyzed retrospectively.We summarized and analyzed the therapeutic effect by nasal endoscopic surgery and the role of drug therapy during the periopreative period,such as antibiotics,mucus clearing medicine,nasal atomizing hormone,and nasal irrigation,etc.Results All of the 86 patients were cured by the one-time treatment without recurrence during the follow-up of 1-3 years.Drug therapy during the perioperative period facilitated declining of hemorrhage,relief of postoperative mucosal edema,decrease of secretion,and shortness of the disease course.Conclusion Radical elimination of lesion sites under nasal endoscope is a safe and effective therapeutic approach for non-invasive fungal sinusitis,and reasonable drug therapy during the perioperative period plays an active role.
