PTTG、VEGF-C 基因过表达与非小细胞肺癌发生、发展的关系及其临床意义

摘    要:目的:检测非小细胞肺癌(NSCLC)组织中垂体瘤转化基因(PTTG)及血管内皮生长因子 C(VEGF C)的表达和微淋巴管密度值(LMVD),探讨它们与NSCLC发生、发展的关系及其临床意义。方法:实时荧光定量PCR和免疫组化分别检测NSCLC中 PTTG、VEGF C mRNA 和蛋白的表达,结合临床病理特征及预后进行分析。同时,对随访资料进行生存分析,绘制生存率曲线,探讨PTTG、VEGF C 对肺癌患者预后的影响。结果:PTTG、VEGF C mRNA 和LMVD值在不同性质肺组织中的表达差别均有统计学意义(P<0.05),在不同年龄、性别、是否吸烟、肿瘤大小、组织学类型及分化程度组间差别无统计学意义(P>0.05),在TNM分期、淋巴结转移与否及预后组间差别有统计学意义(P<0.05)。PTTG、VEGF C蛋白表达在淋巴结转移与否及预后组间差别有统计学意义(P<0.05)。生存率曲线显示,PTTG、VEGF C 高表达者生存时间均短于低/无表达者(P=0.030,0.027,n=65)。PTTG表达与VEGF C、LMVD密切相关,随着PTTG表达增加,VEGF C分级及LMVD值亦增加。结论:PTTG、VEGF C 的过表达促使肿瘤微淋巴管生成,进而促进了肿瘤细胞淋巴结转移;PTTG和VEGF C表达可作为判断NSCLC生物学行为及肺癌患者预后的良好指标;VEGF C有望成为肺癌抗淋巴管治疗的新靶点。

Abstract:Objective: To investigate the lymphatic microvessel density and expressions of pituitary tumor transforming gene and vascular endothelial growth factor C in non-small cell lung cancer(NSCLC),and explore their correlations to clinicopathologic features of NSCLC.Methods:Real-time fluorescence quantitative PCR and immunohistochemistry method were used to detect the expressions of PTTG,VEGF-C and LMVD in NSCLC.We also investigated the effects of PTTG and VEGF-C on the prognosis of patients with NSCLC by drawing Kaplan-Meier curves.Results: There were higher expressions of PTTG,VEGF-C and LMVD in NSCLC tissues than in adjacent tissues and benign lesions(P<(0.05)).There were no significant differences between the expressions of PTTG,VEGF-C and age,sex,smoking,and size,histological type and differentiation of tumor(P>0.05),but there were statistical relationships between their expressions and TNM stage,lymph node metastasis and prognosis(P<0.05).The Kaplan-Meier curves showed that patients with low or no expressions of PTTG or VEGF-C survived longer than those with high expressions.The expression of PTTG waspositively correlated with VEGF-C expression and LMVD.Conclusion: Tumor lymph angiogenesis is enhanced by the overexpression of PTTG and VEGF-C;PTTG and VEGF-C can be used as two useful markers to judge the biological behavior and the prognosis of lung cancer;VEGF-C can be used as a new target of anti-lymphatic therapy for lung cancer.
