髓鞘碱性蛋白和血清酶评估小儿病毒性脑炎脑损伤的价值

摘    要:目的 探讨血清酶和髓鞘碱性蛋白 (MBP)评估小儿病毒性脑炎脑损伤程度及预后的价值。方法  43例患儿按有无惊厥、有无意识障碍分组 ,采用多功能生化分析仪检测血清酶 ,简易MBPELISA法检测MBP ,组间进行对比分析 ,随访预后情况。结果 有惊厥组和有意识障碍组的血清酶和MBP显著高于无惊厥组和无意识障碍组 (P <0 .0 1) ;血清酶和MBP增高组后遗症发生率高 (P <0 .0 5 )。结论 血清酶和MBP均可反映病毒性脑炎的神经系统损伤程度 ,MBP较血清酶更能反应神经系统损伤 ,可作为病毒性脑炎神经系统损伤及预后判断的一项生化指标。

Abstract:Objective To evaluate the degree of nervous tissue injury of children with viral encephalitis by the change of enzyme and of myelin basic protein in serum( MBP) and its prognostic significance.Methods Forty three children with viral encephalitis were divided into two groups according to the symptoms of the convulsion and the disturbance of conciousness . The enzyme and MBP in serum were detected respectively by various ways. Comparsion between groups analysis was made , and its prognostic significance was followed-up .Results The change of enzyme and MBP in serum of patients with the symptoms of the convulsion and the disturbance of conciousness was signficantly higher than those without the symptoms of convulsion or disturbance of consciousness(P<0.01) .The higher the occurrance rate of the sequela was seen in patients with higher levels of the enzyme and MBP in serum(P<0.05) .Conclusion Both enzyme and MBP in serum may reflect the degree of damage of nervious tissue in viral encephalitis ,but MBP is better than seroenzymes in reflecting brain injury of viral encephalitis . So MBP may be regarded as a biochemical index in reflecting brain injury and judging prognostic.
