免疫细胞亚群与恶性血液病患者预后关系的探讨

摘    要:目的探讨恶性血液病患者化疗前后免疫功能状态变化及临床意义.方法 95例恶性血液病患者随机分为A、B两组,A组49例,用胸腺肽 化疗,B组46例用单独化疗,治疗前应用单克隆抗体和流式细胞术测定患者的CD3、CD4、CD4/CD8和NK值,以其住院的80例非肿瘤病人作为对照组.结果 (1)恶性血液病患者各项免疫指标均低于对照组;(2)晚期和复发型恶性血液病患者各项免疫指标均低于初发型恶性血液病;(3)急性白血病各项免疫指标明显低于其它恶性血液病;(4)治疗后A、B两组比较:A组的NK细胞活性明显高于B组,A组治疗后CD3、CD4、CD4/CD8、NK细胞活性明显高于治疗前.结论免疫细胞亚群在恶性血液病患者中受到抑制并随病情程度而进一步加剧,其中急性白血病患者受抑更甚,胸腺肽能明显提高恶性血液病患者的细胞免疫功能,尤其对NK细胞,是一种低毒高效的生物反应调节剂.

Abstract:Objective To explore the changes of the immunological function in patients with malignat hemopathy before and after chemotherapy. Methods The application of Monoclonal antibody and flow cytometry . Results (1) the immune indexes of malignant hemopathic patient were lower than those of the control group. (2) The immune indexes of the advanced and recurrent malignant hemopathic patients were lower than those when first symptomized. (3) The immune indexes of the acute Leukemia were obviously lower than these of any other malignant hemopathy. (4) After treatment, the NK cell activity in A group was significantly greater than that of B group; the peripheral blood of T cell subgroup and NK cell activity after treatment were significatly greater than those before the treatment. Conclusion The suppression in acute leukemia patients is more significant. The TP can significantly improve the cell immunity function of malignat hemopathic patient, in particular, for NK cell, it is a highly effective biological-response-regulator without known side effect.
