血清胆汁酸谱对肝癌无水乙醇注射治疗监测的临床价值

目的:探讨血清胆汁酸谱检测对肝内肿瘤无水乙醇注射治疗的指导意义。方法:应用高效液相色谱法对无水乙醇瘤内注射或术中同时行肝动脉化疗栓塞治疗前、后原发性肝癌及肝硬化、正常对照的9种血清胆汁酸进行检测。结果:肝癌血清总胆汁酸和甘氨结合型胆汁酸高于正常对照组(P<0.05),而明显低于肝硬化组(P<0.01)。治疗后胆酸/鹅脱氧胆酸比值一过性增高;甘氨结合型胆汁酸/总胆汁酸比值下降;总胆汁酸增高,第7天尚未恢复。结论:血清胆汁酸谱作为肝功能有效指标对肝癌治疗监测有一定价值。

Clinical Values of Individual Bile Acids in Serum to Primary Hepatic Carcinoma Treated by Absolute Ethanol Injection

Objective: Our aim was to study serum levels of individual bile acids in evaluation of intrahepatic tumor treated by absolute ethanol injection. Methods: Using high performance liquid chromatography, we measured serum levels of 9 individual bile acids in patients with primary hepatic carcinoma (PHC) , who were treated before and after absolute ethanol injection or both hepatic artery chemotherapy embolism and absolute ethanol injection during operation, patients with liver cirrhosis (LC) , and healthy persons. Resutes: Serum levels of total bile acids and glycocholic acids in patients with PHC were significantly higher than those of healthy persons( P <0. 05) and lower than those with LC( P <0.01). Ratio of serum levels of cholic acids to chenodeoxycholic acids increased temporarily after treatment . Ratio of serum glycine-conjugated bile acids to total bile acids decreased. Serum total bile acids increased, and didn't recover until the 7th day after treatment. Conclusion: Serum levels of individual bile acids as an effective indicator of liver function are valubale to monitor of PHC therapy.
