24252例消化性溃疡临床调查分析

目的了解南宁市消化性溃疡的临床特点。方法对南宁市17家医院1992～1997年间经胃镜检查诊断为消化性溃疡的24252病例进行回顾性分析。结果（1）6年间从104121例胃镜检查共检出消化性溃疡24252例,检出率为23．29％,其中男性18215例,女性6037例,男女之比约为3：1。检出十二指肠溃疡（DU）21310例次（87．87％）,胃溃疡（GU）2349例（9．69％）,复合性溃疡（CU）577例（2．43％）,食管溃疡16例（0．06％）。（2）南宁市消化性溃疡年发病年龄男性平均为38．81岁,女性为41．03岁。高发年龄段在20-49岁年龄段,共17305例,占所有病例数的71．35％。（3）24252消化性溃疡病例中共检出溃疡共28963个,其中十二肠溃疡（DU）25233个（87．122％）,胃溃疡（GU）3714个（12．823％）。胃溃疡部位数中,胃窦（45．90％）〉胃角（21．22％）〉胃体（19．52％）〉幽门管（8．27％）〉胃底（4．15％）〉残胃（0．94％）,差异有统计学意义（P〈0．05）;十二指肠溃疡部位数中,球部（98．01％）,球后（1．99％）,两者比较差异有统计学意义（P〈0．05）。球部前壁（47．16％）〉大弯（24．90％）〉小弯（16．67％）〉后壁（11．27％）。（4）11-4月消化性溃疡月平均检出率为24．2％-28．8％,峰值在1月（28．8％）,5-10月为20．0％-22．6％,谷值在6月（20．0％）;冬季（11-1月）检出率为26．48％,春季（2-4月）24．98％,夏季（5-7月）21．10％,秋季（8～10月）为20．91％。四季消化性溃疡检出率比较差异有统计学意义（P〈0．05）。结论南宁市消化性溃疡发病年龄男性平均为38．81岁,女性为41．03岁,发病人群主要集中在20-49岁,男性发病率明显高于女性。十二指肠溃疡发病明显高于胃溃疡。胃溃疡部位分布：胃窦〉胃角〉胃体〉幽门管;十二指肠溃疡部位分布：球部〉?

study of 24 252 cases of peptic ulcer

Objective To find out the clinical characteristics of peptic ulcer （PU）. Methods Analysis of 24 252 PU cases during 1992 - 1997 from 17 hospitals in Narming. Results Among 104 121 cases which took gastroseope procedure,24 252 eases were with PU,the recall rate was 23.29%. Male and female patients were 18 215 and 6 037 ,respectively. The ratio was 3 ： 1 ;21 310 eases （ 87.87% ） were duodenal ulcer（DU） ,2 349 cases （9.69%） were gastric ulcer （GU） ,577cases （2.43%） were compound ulcer （CU）, 16 eases （0.06%） were esophageal neoplasms. The average age of male and female eases were 38.81 and 41.03 years ,respectively. 17 305 eases were 20- 49 years （71.35%）. 28 963 ulcers were found in 24 252 cases,in which DU was 25 233 （87. 122% ）,GU was 3 714 （12.823%）. GU at gastrie antrum （45.90%） 〉 gastric angle （21.22%） 〉 gastric body （19.52%） 〉 pylorie canal （8.27%） 〉 gastric fundus （4.15%） ;DU at duodenal bulb （98.01%） were more than posterior duodenal bulb（ 19.89% ）, （P 〈0.05）. DU on anterior wall of duodenal bulb （47.16%） 〉 greater curvature wall （ 24.90% ） 〉 lesser curvature wall （ 16.67% ） 〉 posterior wall （ 11.27% ）. The detection rate of PU from November to April was 24.2% - 28.8% , the peak value presented in January （ 28.8% ） , and from May to October was 20.0% - 22.6% ,the minimum value was in June （20.0%）. The recall rate in winter （ November to January） was 26.48%, in spring （February to April） was 24.98% ,in summer （May to July） was 21.10% and in autumn （August to October） was 20.91%. Conclusion The amean age of male and female cases were 38.81 and 41.03 years, respectively. Morbidity in male was higher than that of female. Most eases were at 20 - 49 years age group. Morbidity of DU was higher than that of GU. GU at gastric antrum 〉 gastric angle 〉 gastric body 〉 pyloric canal 〉 gastric fundus, ulcers at duodenal bulb were more than that of posterior duodenal bulb. Ulcers at
