多层螺旋CT增强结合后处理技术在胆系结石中的临床应用价值

摘    要:目的探讨多层螺旋CT增强结合后处理技术在胆系结石中的临床应用价值。方法收集经手术取石证实的胆系结石患者139例。所有患者均进行多层螺旋CT平扫、增强并结合后处理技术检查和超声检查。回顾性分析所得影像资料,并比较3种检查方法对胆系结石的检出率。采用Pearsonχ^2检验进行统计学处理。结果多层螺旋CT平扫结合后处理技术与增强结合后处理技术比较,在肝内胆管结石、胆囊结石及胆总管结石的检出率均差异有统计学意义(χ^2=6.609,P=0.010;χ^2=5.471,P=0.019;χ2=5.891,P=0.015);多层螺旋CT增强结合后处理技术与超声比较,在胆总管结石的检出率差异有统计学意义(χ^2=34.159,P<0.01),在肝内胆管结石、胆囊结石的检出率差异无统计学意义(χ^2=0.213,P=0.644;χ^2=0.433,P=0.510)。结论多层螺旋CT增强结合后处理技术诊断胆总管结石的效果优于超声,可提高胆系结石检出率,能获得胆道系统更加完整的信息,为胆系结石治疗提供了一个安全、精准、高效的治疗措施。

Abstract:Objective To investigate the clinical applications of contrast enhanced multidetector CT(ceMDCT)combined with post-processing techniques in biliary stone.Methods One hundred and thirty-nine patients with biliary stones were enrolled in this study and the diagnosis was confirmed by surgical pathology.All patients were examined by MDCT scan,enhancement,combined with post-processing techniques and ultrasound(US).The images obtained from the three modalities were analyzed retrospectively,and the detectable rate for biliary stone of ceMDCT combined with post-processing techniques was compared with another modality.For statistical analysis,a Pearson χ^2 test was used for comparison.Results For intrahepatic bile duct stone,gallbladder stone and common bile duct stone,the difference of detectable rate between MDCT scan combined with post-processing techniques and ceMDCTcombined with post-processing techniques was significant(χ^2=6.609,P=0.010;χ^2=5.471,P=0.019;χ2=5.891,P=0.015);For common bile duct stone,the difference between MDCT combined with post-processing techniques and US there was significant difference(χ^2=34.159,P<0.01).However,no statistically significant difference was seen for in-trahepatic bile duct stone and gallbladder stone(χ^2=0.213,P=0.644;χ^2=0.433,P=0.510).Conclusion CeMDCT combined with post-processing techniques is superior to US for diagnosis of common bile duct stone,which can im-prove the detectable rate of biliary stone and obtain more complete information of biliary system.It could provide safe,precise and efficient therapeutic measures for biliary stone.
