经脐单孔腹腔镜胆囊切除术400例报告

目的:探讨经脐单孔腹腔镜胆囊切除术(laparoscopic cholecystectomy,LC)的可行性、临床价值及并发症的预防措施。方法:回顾分析400例经脐单孔LC的临床资料。结果:397例成功完成经脐单孔LC,3例因严重粘连、术野暴露困难中转三孔法LC。手术时间平均20 min;创面出血量<5 ml,术后当天或次日患者可下地活动,并进流质饮食。术后3～5 d出院,无并发症发生。随访1～36个月无任何不良反应。结论:经脐单孔LC技术难度较大,但不难掌握,仔细操作可避免并发症的发生,具有更加微创、腹部美容的效果,值得临床推广。

Transumbilical single-port laparoscopic cholecystectomy : a report of 400 cases

Objective:To evaluate the feasibility,clinical value and prevention of complication of transumbilical single-port laparoscopic cholecystectomy.Methods:Clinical data of 400 patients who underwent transumbilical single-port laparoscopic cholecystectomy were analyzed retrospectively.Results:Transumbilical single-port laparoscopic cholecystectomy was performed successfully in 397 patients.Three patients were converted to three-port laparoscopic cholecystectomy due to severe adhesion and poor exposure.Mean operation time was 20 min.Intraoperative blood loss ranged from 0 to 5ml.Ambulation was implemented on the operative day or the day after operation,and liquid diet was resumed.All patients were discharged on postoperative day 3 to 5.No complication was recorded after surgery.No discomfort was complained of during follow-up(1-36 months).Conclusions:Transumbilical single-port laparoscopic cholecystectomy is safe and feasible with good clinical value.Although this technique is difficult,surgeons can master it easily.Complications can be avoided by careful operation.Although it needs some technique training,it is associated minimal invasiveness and good cosmetic results.It should be popularized in future.
