负压封闭引流技术治疗四肢创伤复杂创面的临床应用研究

目的探讨负压封闭引流（vacuum sealing drainage,VSD）技术治疗在四肢创伤复杂创面中的临床应用价值。方法 回顾性分析我院自2009年6月-2010年6月收治的100例创伤性四肢骨折患者的临床资料。100例患者根据治疗方式分为治疗组和对照组,治疗组采用VSD治疗,对照组采用常换药治疗。结果治疗后,治疗组和对照组创面面积分别为（26.52±10.19）cm2和（43.80±14.40）cm2,与治疗前比较差异有统计学意义（P〈0.05）;且治疗组与对照组比较差异有统计学意义（P〈0.05）。治疗组临床疗效总有效率为98.0%,显著高于对照组（64.0%）,相比较差异有统计学意义（P〈0.05）。治疗组并发症发生率为16.0%,显著低于对照组（46.0%）,相比较差异有统计学意义（P〈0.05）。治疗组住院时间为（18.19±5.68）d,医疗费用为（16 359.48±1 759.89）元,显著低于对照组（P〈0.05）。结论 VSD是治疗四肢创伤复杂创面的理想方法,值得临床推广应用。

Clinical Application of Vacuum Sealing Drainage Technique in Traumas of Extremities with Complex Wound

Objective To explore the clinical application value of vacuum sealing drainage(VSD) technique in complex extremity trauma. Methods The clinical data of 100 patients with complex extremity trauma in our hospital from June,2009 to June,2010 were retrospectively analyzed.100 patients were divided into treatment group and control group according to surgical methods.The patients in treatment group were received VSD therapy,while the patients in control group were given the treatment of regular dressing. Results After the treatment,the wound area of treatment group was(26.52±10.19) cm2 and that of control group was(43.80±14.40) cm2,the difference was statistically significant between before and after the treatment(P<0.05);moreover, there were statistically significant differences between treatment group and control group(P<0.05).The total effective rate of treatment group was 98.0%,which was significantly higher than that of control group(64.0%),and the difference was statistically significant(P<0.05).The incidence rate of complications in treatment group was 16.0%,which was significantly lower than that of control group(46.0%),and the difference was statistically significant(P<0.05).The duration of hospitalization in treatment group was(18.19±5.68) days and the medical care expenses were(16,359.48±1,759.89) Yuan,which were significantly lower than those of control group(P<0.05). Conclusions The technique of VSD is an ideal method in treating the traumas of extremities with complex wound.It is worthy of clinical application.
