黄韧带骨化的影像学诊断

摘    要:
作者对21例胸椎黄韧带骨化的影像学表现进行了分析。在侧位X线片上,可见骨化的黄韧带突入椎管,体层摄影则可勾划出黄韧带骨化特征。依阻塞程度不同,脊髓造影片上可呈完全性或不完全性梗阻。在不完全梗阻的侧位片上常提示压迫来自椎管后方。CT扫描显示黄韧带骨化增生,造成椎管狭窄及脊髓压迫。MRI检查除可显示位于椎管后缘的黄韧带呈低信号变化之外,还能显示出骨化灶的范围,从而排作其他原因引起的脊髓外压迫。

Abstract:
The authors analyzed the imaging findings of 21 cases with ossification of the liga-mentum flavum(OLF).The existence of OLF projecting into the spinal canal was identified by later-al radiogram of the spine and the features of OLF were delineated by lateral tomogram. Depending on the degree of obstruction,either a partial or complete block to the flow of contrast material could be demonstrated by a thoracic myelogram. The lateral view in case of partial block disclosed the cause of the obstruction from posterior cornpression. CT scan demonstrated hypertrophy and ossification of the ligamentum flavum,which caused stenosis of the spinal canal and spinal cord compression. On MR irnage,OLF presented as a hypointense signal along the posterior margin of the spinal canal.In particular,MRI could demonstrate the extent of OLF and exclude other extramedullary masses.
