脑挫裂伤患者血清心肌型脂肪酸结合蛋白的变化及临床意义

目的:探讨血清心肌型脂肪酸结合蛋白(H-FABP)在脑挫裂伤患者中的变化及临床意义.方法:40例脑挫裂伤患者按GCS评分进行分组,3～4分8例,5～6分8例,7～8分8例,9～12分8例,13～15分8例,分别于伤后3 h、6 h、12 h、24 h、3 d、5 d、7d、14 d抽取患者的血液,进行血清H-FABP定量检查,同时取8例正常人作为对照.结果:所有脑挫裂伤患者血清H-FABP高于正常人(P<0.01),GCS评分越低,血清H-FABP浓度越高(P<0.05),出现增高的时期越早,持续增高时间越长.结论:H-FABP对早期脑挫裂伤患者病情严重程度及预后判断具有重要的应用价值.

Change and clinical significance of serum heart-type fatty acid-binding protein in brain contusion and laceration

Objective：To study the change and clinic significance of serum heart-type fatty acid-binding protein（H-FABP） in brain contusion and laceration.Methods：40 brain contusion and laceration patients were grouped according to Glasgow Coma Scale （GCS） scores.In groups of GCS scores of 3-4,5-6,7-8,9-12 and 13-15,there were 8 cases in each group.Blood was drawn from the patients after brain damage for 3 h,6 h,12 h,24 h, 3 d, 5 d,7 d and 8 cases as control.Results：All the brain contusion and laceration patients had higher serum H-FABP level than normal （P〈0.01）.The lower of the GCS scores,the serum H-FABP could increase to a higher level, which also appeared earlier and lasted longer （P〈0.05）.Conclusion：H-FABP level shows significant value in evaluating the severity and prognosis of early phase brain contusion and laceration.
