摘    要:目的:观察老年患者下呼吸道革兰阴性杆菌感染常见病原菌的分布及耐药状况.方法:对我院老年患者痰液中分离出的186株革兰阴性杆菌选用15种常用抗生索进行药敏试验.结果:分离出的186株革兰阴性杆菌,主要为:铜绿假单胞菌37株,占19.89%,肺炎克雷伯菌55株,占29.57%,鲍曼不动杆菌18株,占19.9%,阴沟肠杆菌16株,占8.60%,大肠埃希菌34株,占18.28%,嗜麦芽寡养单胞菌9株,占4.84%,其他革兰阴性杆菌17株,占9.14%;耐药性分析显示,革兰阴性杆菌对临床常用抗生素的耐药率最低为亚胺培南,最高为氨苄西林,大部分细菌呈多重耐药.结论:老年患者下呼吸道革兰阴性菌耐药谱广,多重耐药严重.

Abstract:Objective:To explore the distribution and drug resistance of Gram-negative bacilli in senile lower respiratory tract infection.Methods:Drug sensitivity test was performed in 186 strains of Gram-negative bacilli by using 15 kinds of ordinary antibiotics from 2006 to 2009 in our hospita1.Results:A total of 186 strains of Gram-negative bacilli were isolated from senile patients'sputum in the past two years.The most common pathogen was Pseudomonas aeruginosa(37 strains,19.89%),Klebsiella pneumoniae(55 strains,29.57%),Acinetobacter baumanii(18 strains,19.9%),Enterobacter cloacae(16 strains,8.60%),Escherichia coli(34 strains,18.28%),Stenotrophomonas maltophilia(9 strains,4.84%),and 17 strains of the other bacilli.In our surveillance,the highest sensitive antibiotic to Gram-negative bacilli was imipenem,while the lowest was ampicillin.Most of strains were multi-drug resistant.Conclusion:Most of Gram-negative bacilli from lower respiratory tract have wide drug-resistance spectrum with severe resistance to many kinds of antibiotics.
