急性心肌梗死急性早期空腹高血糖与住院新发心房颤动关系的研究

摘    要:
目的 探讨AMI急性早期空腹高血糖与住院新发心房颤动的关系。方法 将来我院就诊的急性心肌梗死患者336例分为两组,对照组(n=195)/空腹高血糖组(n=141);再次分组:心功能KillipⅠ级133例/79例、Ⅱ级52例/37例、Ⅲ~Ⅳ级10例/25例。结果 空腹高血糖组男性、有吸烟史、使用血管紧张素转换酶抑制剂、左心室射血分数较低、糖尿病史、心功能KillipⅡ~Ⅳ级、使用利尿剂、CK-MB较高(P〈0.05);心功能KillipⅠ级男性较低,糖尿病史比例、房颤发生率、Ⅱ级房颤发生率较高(P〈0.05);空腹高血糖是AMI患者住院期间发生房颤的影响因素(P〈0.05)。结论 将AMI空腹高血糖控制在良好水平有利于降低新发心房颤动发生率,对AMI患者疾病预后的改善有积极意义。

Abstract:
Objective To investigate the relationship between fasting hyperglycemia and new-onset atrial fibrillation hospitalized AMI acute. Methods A total of 336 cases of acute myocardial infarction were divided into two groups, control group(n=195)/fasting hyperglycemia group(n=141); Again grouped :heart function Killip I grade(n=133/79), Killip Ⅱ grade (n=52/37),Ⅲ to Ⅳ grade(n=lO/25). Results The proportion of male,patients with a history of smok- ing, using angiotensin-converting enzyme inhibitors,the left ventricular ejection fraction of the fasting hyperglycemia group was lower. The proportion of patients with a history of diabetes, heart function Killip Ⅱ to Ⅳstage,using di- uretics, CK-MB of the fasting hyperglycemia group was higher (P〈O.05); The proportion of male of the fasting hyper- glycemia cardiac function Killip I grade was lower, the proportion of diabetes, the incidence of atrial fibrillation, the incidence of atrial fibrillation of the function Killip Ⅱ grade group was higher(P〈0.05);Fasting hyperglycemia was the influence factors of atrial fibrillation occurred during the AMI hospitalization (P〈0.05). Conclusion AMI fasting hyper- glycemia in a good level of control will help to reduce the incidence of new-onset atrial fibrillation, has a positive meaning in improving the prognosis of patients with AMI.
