丹参注射液预防胸科手术后患者下肢深静脉血栓形成的临床研究

目的：探讨丹参注射液用于预防胸科手术后患者下肢深静脉血栓形成（Deep vein thrombosis,DVT）的效果。方法：将100例胸科食管癌、贲门癌手术患者随机分为用药组（50例）和未用药组（50例）,前瞻性比较术前及术后1周患者血液黏度、凝血酶原时间（PT）、活化的部分凝血活酶时间（APTT）、纤维蛋白原（Fbg）;行双下肢静脉超声检查;并记录有无异常出血、胸腔引流液量有无增加等临床症状。结果：血流变及凝血功能比较：两组患者手术前各项指标比较,无显著性差异（P〉0.05）;用药组手术后各项指标较手术前有下降趋势,但无显著性差异（P〉0.05）。经双下肢静脉超声检查发现,用药组DVT发生率为12%,未用药组DVT发生率为26%,两者比较,有显著性差异（P〈0.05）;治疗期内,两组均未出现异常出血,胸腔引流量稍增多,但无显著性差异（P〉0.05）。结论：丹参注射液能明显改善胸科手术后患者血液流变学的各项指标,降低DVT发生率,用于预防胸科手术后患者DVT可取得较好的疗效,值得进一步推广。

Study on preventive effect of danshen injections against deep vein thrombosis in patients after thoracic surgery

Objective： To investigate the effects of danshen injection on prevention of deep vein thrombosis in patients after thoracic surgery.Methods： All 100 surgical patients with esophageal and cardia cancer were randomly divided into two groups： treatment group（50 cases） and non-treatment group（50 cases）.Before and one week after operation,the blood viscosity,PT,APTT and Fbg were measured and prospectively compared.Lower extremity venous Color Doppler flow imaging was peformed and clinical symptoms（including abnormal bleeding,the amount of thoracic cavity drainage） were recorded.Results： There were no significant differences in the incidence of blood flow change and blood coagulation between the two groups before operation（P0.05）;the indexes of treatment group showed downtrend,but there were no significant differences（P0.05）.By lower extremity venous Color Doppler flow imaging,the incidence of DVT in the treatment group was 12%,non-treatment group was 26%,there were significant differences between the two groups（P0.05）.In treatment period,both groups without abnormal bleeding,the amount of thoracic cavity drainage increased slightly,but there were no significant differences（P0.05）.Conclusion： Danshen injection could obviously improve the blood flow change indexes of patiens after thoracic surgery,reduce and prevent the rate of DVT,so it is worth popularizing.
