| 摘 要： | 本文报导２２例风心病并发房颤的患者，在使用洋地黄过程中，常规心电图均出现二度或三度房室传导阻滞，在用Ｈｏｌｔｅｒ２４小时动态心电监测下，继续任用详地黄，未见房室传导阻滞加重现象，表明常规心电图诊断，房颤合并房室传导阻滞的方法和标准均存在问题，因此，我们建议，在诊断房颤合并房室传导阻滞时，不能用电复律的情况下，使用Ｈｏｌｔｅｒ２４小时动态心电监测，并提出不成熟的标准。 |

| Abstract: | This article reports 22 cases of rheamatic heart diseases(RHD) with atrial fibrillation (AF).All of these cases have Ⅱ°AV block(Ⅱ°AVB)in routine electrocardiogram in theprocess of using digitalis。 When we continue to use digitalis under 24 hours ambalatory mon-itoring We didn’t find the degree of block becomes severe. The result shows that the waysand standards of diagnosing AF with AVB have some problems. We suggested that the 24hours ambulatory electrocardiography should be used for those who have AF with AVB butcan not defibrillate. Therefore the authors proposed their unmatured diagnostic standard. |
