血脂联素和肿瘤坏死因子-α与原发性高血压的关系

目的检测原发性高血压（essential hypertension，EH）患者血浆中脂联素（adiponeetin）和肿瘤坏死因子－α（TNF－α），探讨脂联素和TNF—α与EH的关系。方法67例新发现且未经过治疗的轻、中度EH患者和42例正常人，记录年龄、性别，检测体重指数（BMI）、空腹血糖（FPG）、血脂谱及血压，用放射免疫分析法测定空腹胰岛素（FINS）、血浆脂联素及TNF—α水平。结果①EH组血浆脂联素水平为（17．05±3．08）ng／ml，对照组为（23．05±3．08）ng／ml，EH组较对照组显著降低（P〈0．01）。②EH组血浆TNF—α水平为（26．55±2．55）fmol／ml，显著低于对照组（24．62±2．70）fmol／ml（P〈0．01）。③Pearson相关分析表明，EH组血浆脂联素水平与TNF—α、年龄、PP及SBP呈显著负相关（P〈0．05）。结论低脂联素血症、炎症因子共同作用，可能部分参与了EH的发病过程。

Clinical Studies on Plasma Adiponectin Level,Tumor Necrosis Factor-alpha in Patients with Primary Hypertension

Objective To explore the blood plasma adiponectin, inflammatory factors TNF - α concentrations and the association of essential hypertension. Metheds A total of 67 hypertensive patients, all coincided with 1999 WHO diagnostic standard, never-treated and newly diagnosed was designated as hypertensive group. As a control group, 42 body mass indexmatched normotensive healthy subjects were included randomly. Secondary hypertension, diabetes mellitus, coronary heart disease, heart failure, renal failure, cerebrovascular disease, all acute infection, chronic infection patients were excluded from three groups. All of them were measured height and weight, calculated BMI. Fasting plasma glucose, plasma lipids were measured. Fasting blood plasma were collected and stored at 〈 -20℃. Fasting insulin, tumor necrosis factor - alpha （TNF - α）, circulating adiponectin level were batched measured, they were all analyzed using a radioimmunoassay. Results ①After logarithmic transformation, the value of adiponectin of each group were from heterogeneity of variance to homoscedasticity, it was significant difference between control group and hypertensive group（P 〈 0.01 ）. ②Tumor necrosis factor -alpha （TNF -or）in hypertensive group were conspicuous higher than that in control group, there were significant different among two groups （P 〈0.01）.③Pearson relation analysis showed that adiponectin levels were negatively correlated with the TNF -α （ r = -0.376,P〈0.00l）,age（r= -0.37, P〈0.001）,PP（r= -0.199,P〈0.05） and SBP （r= -0.188, P 〈 0.05 ）in hypertensive group. Conclusions The hypertensive patients followed hypoa-diponectinemia. The reciprocal association of adiponectin and TNF -α level might participate in the development of essential hypertension.
