| 摘 要： | 目的评价巢式PCR在烟曲霉侵袭性感染早期病原学诊断中的应用价值。方法构建烟曲霉播散性感染动物模型,收集不同感染时段动物血液标本20份,分别进行巢式PCR检测和真菌培养,并对感染动物多次进行影像学检查;测定25例临床诊断为侵袭性曲霉病患者的45份血清样本,并与血清半乳甘露聚糖（GM）测定结果进行比较。结果巢式PCR检测疾病模型动物血清曲霉特异基因,标本阳性率为80%（16/20）,最早在感染后的第二天即可检出;而在整个感染过程中,血培养结果均为阴性,影像学检查则在感染后期才提示真菌感染的存在。临床标本的检测结果显示,巢式PCR敏感性和特异度分别为64%和100%。结论巢式PCR具有较好的敏感性及特异度,可用于曲霉深部感染的早期诊断,与血清GM测定联合应用诊断意义更大。
