血清光抑素C与糖尿病肾病的相关性研究

目的旨在研究探讨糖尿病肾病（DN）患者血清胱抑素C（CysC）、血肌酐（Scr）和尿微量白蛋白（MA）水平的变化及意义。方法选取血糖控制达标的糖尿病肾病（CKD2-3期）患者60例,随机分成3组,1组为对照组,未使用阿托伐他汀;2、3组为治疗组分别给与阿托伐他汀20 mg/d、40 mg/d。治疗组给予治疗16周,分别检测对照组、治疗组治疗前、治疗后6、12、16周尿微量白蛋白、血肌酐、血清光抑素。结果治疗组治疗前与对照组比较差异无显著性（P〉0.05）;治疗组经阿托伐他汀治疗6、12、16周后,尿微量白蛋白、血肌酐、血清光抑素明显下降,12周和16周与治疗前水平比较差异有显著性（P〈0.05）,16周与6,12周比较差异亦有显著性（P〈0.05）,但6周与治疗前比较则差异无显著性（P〉0.05）;20 mg/d组与40 mg/d组相比差异有显著性。结论在糖尿病患者肾功能损害中CysC水平与Scr、MA有很好的相关性,是糖尿病肾病的敏感指标。

Study on the serum cystatin C level in patients with diabetic nephropathy

Objective To investigate the clinical significance of determination of serum cystatin C,Scr and urine microalbumin contents in patients with diabetic nephropathy.Methods 60 case of patients with DN（2、3） were randomly divided into 3 groups,the 1group was not used Atorvastatin,the 2and 3group was used Atorvastatin 20 mg/l、40 mg/l.2、3groups cured 16weeks,serum cystatin C,Scr and urine microalbumin were detected at baseline and 6 week late、12 week late、16week late.Results There were no different between 1 and 2、3 before cured;6 week late、12week late、16week late the serum level of MA、Scr、 CysC were declined,12week late、16week late the serum level of MA、Scr、CysC had significance to before cured（P0.05）;16week late the serum level of MA、Scr、CysC had significance to 6week late、12week late（P0.05）;6 week late had no significance to before cured（P0.05）,the 2 group and 3 group had significance（P0.05）.Conclusion Serum levels of CysC are correlated with MA and Scr levels in the diabetic nephropathy.They are sensitive markers diabetic nephropathy.
