| 摘 要： | 背景:骨巨细胞瘤传统治疗方法复发率高,而对其机制的日益了解及肿瘤型假体在骨肿瘤切除中的成功应用,为提高骨巨细胞瘤的治疗效果提供了更广阔的前景.目的:总结肿瘤型假体在骨巨细胞瘤治疗中的作用及该肿瘤目前的治疗进展、发病机制.方法:应用计算机检索2000-01/2010-10 PubMed数据库、中国知网数据库相关文章,中文检索词为"骨巨细胞瘤,研究进展",英文检索词为"giant cell tumor of bone,tumor-type prosthesis,therapy research".共检索到文献475篇,保留符合标准的35篇进行综述.结果与结论:治疗骨巨细胞瘤的传统方法是手术切除,并根据情况对切除后产生的骨缺损进行重建.将各种骨重建方法进行比较得出,应用肿瘤型假体治疗骨巨细胞瘤效果更好,不仅能使患者获得患肢或患椎的即刻稳定性,而且并发症少,相对于其他的重建方式明显降低了假体置入后复发率,实现了肿瘤的广泛切除.但肿瘤型假体的应用并不能完全避免置入后肿瘤的复发.最近针对骨巨细胞瘤形成信号通路的研究取得了成果,为降低置入后复发率及不能手术的患者提供了新疗法. |
