足部浅部真菌感染临床诊断与真菌学检查结果的差异性

目的：了解临床医生仅凭临床表现判断足部浅部真菌感染种类的准确率及临床诊断、真菌镜检和培养结果之间的符合情况。方法：随机选择2007年5月～2007年10月到本科门诊就诊，由我科主治医师以上职称的医师拟诊为足部浅部真菌感染的患者，取皮屑做真菌镜检和培养，计算其真菌学检查阳性率及临床诊断、真菌镜检和培养结果之间的符合率。结果：确诊患者中临床诊断和真菌镜检结果的符合率为61．21％，临床诊断和真菌培养结果的符合率为33．62％，真菌镜检和真菌培养结果同时阳性的概率为68．10％；另外，在确诊患者中皮肤癣菌和念珠菌混合感染率为8．62％。结论：仅根据临床表现判断足部真菌病种类的准确率不高，真菌培养是诊断的金标准；皮肤癣菌和念珠菌混合感染值得重视。

Variability between clinical diagnosis and the results of mycological examinations of foot superficial fungal infections

Objective:To investigate the consistency between the diagnoses made by the senior dermatologists according to clinical manifestations only and that made according to mycological findings.
