Toll样受体4在多发性骨髓瘤患者外周血白细胞的表达及临床意义初探

为了探讨Toll样受体4(Toll-like receptor 4,TLR4)在外周血白细胞上的表达及其在多发性骨髓瘤中的意义。采用流式细胞术检测50例多发性骨髓瘤患者及35名健康对照者外周血淋巴细胞、单核细胞和中性粒细胞上TLR4的表达。结果TLR4在患者外周血淋巴细胞和中性粒细胞中的表达率较低;单核细胞TLR4阳性表达率在多发性骨髓瘤患者组为(24.71%±7.83%),显著低于健康对照组阳性率(43.14%±15.20%),差异有统计学意义(P<0.01),而TLR4在各型多发性骨髓瘤患者组的阳性表达率差异无统计学意义(P>0.05)。TLR4主要表达于外周血单核细胞,在多发性骨髓瘤患者中其阳性率降低,而且TLR4的阳性率高低与多发性骨髓瘤分型无关。表明TLR4及其介导的天然免疫应答与多发性骨髓瘤的发病存在一定的相关性。

Expression of Toll-like receptor 4 in peripheral blood of patients with multiple myeloma and its clinical significance

To observe the differential expression of Toll-like receptor 4(TLR4) in peripheral blood of patients with multiple myeloma(MM) and to evaluate its clinical significance,the expressions of TLR4 in peripheral blood lymphocytes,monocytes and neutrophils of 50 cases of MM and 35 healthy individuals were assayed with flow cytometry.It was found the expression rate of TLR4 in peripheral blood monocytes of patients was high,but those in lymphocytes and neutrophils of patients were rather low.However,the positive expression rate of monocytes in patients with MM was significantly lower than that of healthy controls with the statistic difference(24.71%±7.84% vs 43.14%±15.20%,P<0.01).It is evident that TLR4 mainly expresses in peripheral blood monocytes of patients with multiple myeloma,but its positive expression rate is lower than that of healthy controls,indicating that the innate immune responses mediated by TLR4 signaling pathway is associated to the pathogenesis of multiple myeloma.
