不同皮肤准备方式对骨科手术感染影响的实验研究

目的研究不同皮肤准备方式对骨科术后感染发生的影响,探讨骨科术前简化备皮方式的可行性。方法选用Wistar大鼠40只,制备成各部位骨折切开复位内固定动物模型,其对侧肢体作为对照,实验侧肢体于术晨剪去影响手术操作的毛发,用肥皂水清洗手术部位皮肤;对照侧肢体按传统骨科备皮方法,连续3d准备皮肤,3次更换无菌巾包裹;分别于手术消毒前及术后15h在手术部位取样做细菌培养,观察术后切口感染情况。结果实验侧与对照侧肢体在手术消毒前细菌培养比较差异有显著性(P<0.05);在术后15h细菌培养和术后切口感染率比较差异无显著性(P>0.05)。结论简化骨科术前备皮方式并不增加术后切口感染的风险,且可减少对皮肤的损伤。

Experimental Study of Differentways of Skin Preparation on the Incidence of Postoperative Infections in Bone Operation

Objective To study the influence of different ways of skin preparation on the incidence of postoperative infection in bone operation,and explore the possibility of improving the method of skin preparation in bone operation. Methods Forty Wistar rats were used to prepare various bone fracture models. The control limbs were prepared by traditionalway of skin preparation for bone operation. The experimental limbs were shaven cloan and then were washed with soapsuds. Samples from operative sites before sterilization and 15 hours after operation were takenforbacteria culture. Results There was no differences in bacteria culture of the skin 15 hours after operation and from the incisional site between the control and experimental limbs (P>0.05). But there was significant difference in bacteria culture before sterilization between the two groups (P<0.05). Conclusion Simplified skin preparation before bone operation did not increase infection incidence of operative incision .
