瑞芬太尼复合七氟醚在腹腔镜手术中的应用(附30例报告)

目的探讨瑞芬太尼-七氟醚复合麻醉在腹腔镜手术中的可行性、安全性和有效性。方法 ASAI~Ⅱ级择期行腹腔镜手术病人60例,随机分为芬太尼-丙泊酚组（A组）和瑞芬太尼-七氟醚组（B组）,每组30例。观察两组病人手术前后多时点的SBP、DBP、HR、术中血液动力学的变化,术毕停药后病人恢复情况。结果在手术切皮时（T1）和气腹2m in时（T2）,A组的SBP高于B组（P〈0.05）,B组病人术后呼吸恢复时间、睁眼时间、拔管时间明显短于A组（P〈0.05）。结论瑞芬太尼复合七氟醚用于腹腔镜手术能提供更平稳的血液动力学及更快的麻醉恢复时间,可控性佳,是理想的麻醉方法。

Application of Remifentanil Combined with Sevoflurane Anesthesia in Laparoscopic Operation(Report of 30 Cases)

Objective To explore the feasibility,security and effectiveness of remifentanil combined with sevoflurane anesthesia in laparoscopic operation.Methods The 60 cases（ASAI Ⅰ~Ⅱ） undergoing laparoscopic operation were randomly divided into two groups with 30 each：Group A（fentanyl combined with propofol） and Group B（remifentanil combined with sevoflurane）.SBP,DBP and HR of the two groups at different times before and after the operation,blooddynamics during the operation as well as the recovery of the patients after the operation were observed and recorded.Results During skin incision（T1） and two minutes after pneumoperitoneum（T2） SBP of group A was higher than group B（P0.05）,the time for breath recovery,eyes opening and tube drawing of group B were much shorter than group A.Conclusion Remifentanil combined with sevoflurane anesthesia in laparoscopic operation is ideal and good at controllability which can provide more stable blooddynamics and more rapid anesthesia recovery.
