乌鲁木齐地区259例耐药肺结核患者分布情况分析

摘    要:
目的:了解乌鲁木齐市耐药肺结核患者人群分布情况。方法:将2011年5月1日~2012年9月30日间,乌鲁木齐市七区一县疾控中心结核病防治科收治的肺结核患者标本用罗氏管进行分枝杆菌分离培养,培养阳性菌株采用比例法对8种抗结核药物:异烟肼、链霉素、乙胺丁醇、利福平、氧氟沙星、卡那霉素、阿米卡星、卷曲霉素进行药物敏感性试验。并对其耐药情况进行统计分析。结果:259例肺结核患者进行药敏试验,其中男性154例,女性105例;年龄分布在14~86岁之间,平均年龄40.8岁。耐药患者71例,耐药率男、女患者分别为29.2%和24.8%;按照年龄分组:患者≤18岁耐药率为21.1%,19~59岁患者耐药率29.7%,患者≥60岁耐药率21.8%;民族分布:汉族患者耐药率25.5%,维吾尔族患者耐药率34.2%,回族患者耐药率18.2%;性别、年龄、民族耐药率经统计分析,差别无统计学意义(P>0.05)。初、复治患者耐药率分别为26.4%和37.5%;耐多药率分别为5.96%和25%。结论:乌鲁木齐地区分支杆菌的耐药性与性别、年龄、族别均无关,但是耐多药率与治疗史密切相关,复治患者远高于初治患者。

Abstract:
Objective : To realize the population distribution of drug resistance tuberculosis in Urumqi Methods Tuberculosis of seven areas one county CDC in Urumqi of 2011.5.1-2012.9.30,specimens were cultured with L-J medium, culture positive isolates using the Drug sensitive test of 8 anti-tuberculosis drugs: INH,RFP,EMB,SM, OFX,KM,AK and CPM. And make statistical analysis of the drug resistance. Results 259 cases of pulmonary tuberculosis patient with Drug sensitive test. 71 cases of drug resistant,Male 154 cases, female 105 cases, Total drug resistance rate were 29.2% and 24.8%;Age distribution between the age of 14-86 years old, average age was 40.8 years old. The age group ≤ 18 years of resistance rate of 21.1%, 19-59 years old resistance rate of 29.7%, 21.8% resistance rate ≥ 60 age group ;The ethnic distribution : Han resistance rate of 25.5%, the Uyghur resistance rate of 34.2%, the Hui resistance rate of 18.2%; Gender ,Age, National resistance rates by statistical analysis, the difference was not statistically significant (P〉 0.05). The Initial treatment and retreatment resistance rates were 26.4% and 37.5%, multi-drug resistant rate of 5.96% and 25%, respectively. Conclusion Drug resistance of Mycobacterium in Gender, Age and Nation were independent in Urumqi. But multi-drug resistant rate related to the treatment. Retreatment patients is much higher than the initial treatment.
