心肌致密化不全与原发性扩张型心肌病超声心动图的对比分析

目的探索心肌致密化不全(NVM)与扩张型心肌病(DCM)彩色多普勒超声心动图形态学鉴别。方法超声心动图检查法以心尖及剑突下四腔观、左心室长轴二腔观和左心室短轴观为主进行多切面检查,对比分析21例NVM及30例DCM的超声心动图资料,重点观察心内膜及心脏腔室结构。结果21例NVM患者超声心动图检查均可见受累心腔内异常隆突,粗大的肌小梁和其间深陷的隐窝影像。心内膜呈网状改变,内膜不光滑为其特点。30例DCM患者除1例在心尖区可见粗大肌小梁外,余29例心内膜均光滑。二者大心腔,射血分数(EF)降低,搏幅弥漫性减弱等彩色超声心动图表现无统计学差异,而心内膜的改变有明显的统计学意义。结论NVM具有特征性超声心动图改变,可区别于原发性DCM,超声心动图是二者鉴别的有价值的无创检查手段。

Comparison of the echocardiographic morphology in patients with noncompaction of ventricular myocardium and dilated cardiomyopathy

Object To investigate the diagnostic methods of echocardiographic morphology in patient with noncompaction of ventricular myocardium(NVM) and dilated cardiomyopathy(DCM).Methods Echocardiographic examination including four-chamber view,two-chamber view and short axis view of left ventricular.were taken The echocardiographic findings of 21 patients with NVM and 30 pati ents with DCM were compared the str ucture of ventricular cavity and endocardium.Results All 21 patients with NVM showed prominent excessive myocardial trabeculations and deep intratrabecular recesses in the affected ventricular cavity.Endocardium of 30 patients with DCM was smooth except 1 patient with DCM,whose endocardium of heart apex had excessive myocardial trabeculations.All patients with NVM or DCM ce had ventricular dilatation,EF decreace,and the wall motion abnormalities.There were no difference in statistics.Conclusion NVM has characteristic echocardiographic manifestations which are different from those of DCM.Echocardiography is a valuable noninvasive technology in differential diognosis of NVM and DCM.
