2型糖尿病患者高血压与糖尿病性神经病变的关系

摘    要:目的 探讨2型糖尿病患者高血压与糖尿病性神经病变的关系。方法 利用心自主神经功能检测系统和神经电生理检测仪对107例(高血压组52例,非高血压组55例)2型糖尿病患者的心自主神经功能和肢体的末梢神经传导速度、皮肤痛温觉、振动沉进行测定,以判断心自主神经病变和末梢神经病变。结果 两组间末梢神经功能和心自主神经功能各指标除心的是距频谱分析的高频值外差异均无显著性(P<0.05)。Logistic回归分析显示高血压与心自主神经病变显著相关(P<0.01),而与末梢神经病变无显著相关。结论 2型糖尿病患者高血压是心自主神经病变发病的危险因素,而与末梢神经病变无明显关系。

Abstract:Objective To investigate the relation between hypertension and diabetic neuropathy in patients with type 2 diabetes mellitus.Methods 107 patients with type 2 diabetes mellitus were investigated.The autonomic tests{coefficient of variation (CV),power spectrum in the low-frequency and high-frequency bands at rest,mean circular resultant (MCR) of vector analysis during deep breathing,the 30:15 ratio to standing,Valsalva ratio and postural change in systolic blood pressure} for the diagnosis of cardiovascular autonomic diabetic neuropathy (CADN) and peripheral nerve functions {motor and sensory nerve conduction velocities in the median,malleolar vibration perception threshold, and thermal perception thresholds} for the diagnosis of peripheral neuropathy (PNP) were performed in each patient using computer-based technique.Results There were no significant differences of the results of all measure except, the spectral analysis of HRV in the high frequency band(P<0.05)between groups. Logistic analysis showed that there was significant association of hypertension with cardiovascular autonomic neuropathy (P<0.01),but not with peripheral neuropathy.Conclusion Hypertension is a major risk factor in the development of diabetic cardiovascular autonomic neuropathy and it is not related to peripheral neuropathy in type 2 diabetes.
