妊娠合并乳腺癌6例的临床特点及妊娠结局分析

目的 分析妊娠合并乳腺癌的临床特点及预后,探讨合理的诊疗方案.方法 收集6例妊娠合并乳腺癌患者的临床资料,分析其生物学特点、治疗方法及孕产情况.结果 妊娠合并乳腺癌患者共6例,发病率为6.094/100000例,其中,5例为浸润癌,1例为原位癌伴浸润;Ⅲa期4例,Ⅲc期1例,Ⅳ期1例.妊娠期手术治疗3例,均为局部麻醉下的保留乳房手术;妊娠中、晚期化疗2例,1例妊娠期采用CAF方案进行化疗,1例妊娠期采用AT方案进行化疗,产后均停止哺乳,继续化疗结束后行手术+放疗进行治疗.共出生4例新生儿,其中,早产1例,无异常;足月产3例,无异常,至今均体健.目前,妊娠晚期产检者2例,胎儿均无异常.结论 妊娠合并乳腺癌的发病率很低(正常人群:31.08/100000),恶性程度偏高,肿瘤分期偏晚.经肿瘤手术或化疗后,对孕产妇继续随诊,目前,均无复发或恶化,产检胎儿均正常,无并发症;新生儿无异常,至今体健.

Analysis of the clinical characteristics and pregnancy outcomes in 6 cases of breast cancer during pregnancy

Objective To analyze the clinical characteristics and prognosis of breast cancer during pregnancy,and to explore the reasonable strategy for diagnosis and treatment.Method The clinical profile of 6 cases of breast cancer dur-ing pregnancy were collected and analyzed for the biological characteristics,therapy and pregnancy outcomes.Result 6 cases of breast cancer during pregnancy were included in the study,the incidence rate was 6.094/100 000,in the 6 cases, 5 were invasive carcinoma and 1 was carcinoma in situ with invasion;there were 4 cases at stageⅢa,1 atⅢc and 1 at stageⅣ.3 cases were administered with breast-preserving surgery under local anesthesia;chemotherapy was performed in 2 cases during the middle or late stage of pregnancy,1 case was treated with CAF chemotherapy and 1 with AT chemo-therapy.All patients stopped lactation after delivery,while surgery and radiation were given after continued chemothera-py.There were a total of 4 newborns,in which 1 case was premature but normal;and the other 3 were full-term healthy in-fants,who are physically normal to now.At present,2 cases were diagnosed in the late stage of pregnancy,and there were no abnormalities observed in the fetus. Conclusion The incidence of breast cancer during pregnancy is relatively low (31.08/100 000 in normal populations),which usually is highly malignant and in advanced stage.After surgery or chemo-therapy,no relapse or progression was observed during follow up,all fetus are in good conditions,with no complications;and newborns are also healthy.
