健脾祛瘀合剂联合胃镜下电凝电切术治疗老年胃息肉

目的观察健脾祛瘀合剂联合胃镜下电凝电切术对老年胃息肉的治疗作用。方法选取我院2012年1月—2013年1月间收治的老年胃息肉患者60例,随机分为观察组与对照组,2组均行胃镜下息肉电凝电切术治疗,术后给予雷贝拉唑钠肠溶胶囊口服;观察组在上述治疗的基础上加服健脾祛瘀合剂治疗,2组患者治疗1疗程后行胃镜取手术部分胃黏膜组织进行病理活检,观察治疗前后胃黏膜病理积分改善情况;并对比2组患者中医证候积分改善及临床疗效。结果观察组治疗1疗程后治愈率为100%,对照组为80.0%,组间比较差异有统计学意义(P<0.01);观察组治疗后中医证候各项积分改善优于对照组(P<0.05);治疗1疗程后胃镜检查显示观察组胃黏膜病理积分、炎症积分改善均显著优于对照组,组间比较差异有统计学意义(P<0.05);2组患者术后6个月随访胃息肉复发率比较无统计学差异(P>0.05)。结论健脾祛瘀合剂联合胃镜下电凝电切术治疗胃息肉,可有效改善患者症状、促进黏膜修复、缓解黏膜慢性炎症。

Effects of Jianpi Quyu Mixture combined with electrocoagulation and electrocision under gastroscope on the treatment of elderly gastric polyps

Objective To observe the curative effects of Jianpi Quyu Mixture combined with electrocoagulation and electrocision under gastroscope on the treatment of elderly gastric polyps.Methods A total of 60 elderly patients with gastric polyp admitted and treated in our hospital from 2012.1 to 2013.1 were randomly divided into observation group and control group,both of the two groups were performed with polyps coagulation treatment under electric gastroscope,followed by oral treatment of rabeprazole azole sodium enteric capsules after operation;the observation group was additionally given Spleen dispelling stasis mixture treatment on the basis of the above treatment.After 1 course of treatment,gastric mucosal tissues from two groups of patients were extracted for gastroscope pathology biopsy,improvements of pathological score of gastric mucosa before and after treatment were observed,and improvements of TCM syndrome score and clinical curative effect of the patients in the two groups were compared.Results After 1 course of treatment,the cure rate was 100% in the observation group and 80.0% in the control group,and the differences between groups was statistically significant（P〈0.01）.After treatment,the improvements of the integral TCM syndrome scores in the observation group were better than those in the control group（P〈0.05）.After 1 course of treatment,gastroscopy showed that pathological score of gastric mucosa and improvements of inflammation scores in the observation group were significantly higher than those in the control group,and the differences between groups was statistically significant（P〉0.05）.Conclusion Using Jianpi Quyu Mixture combined with electrocoagulation and electrocision under gastroscope to treat elderly gastric polyps can effectively improve the patients＇ symptoms,promote mucosal repairment,and alleviate mucosal chronic inflammation,and is an effective therapy for treatment of gastric polyps with distinct curative effect.
