替加氟联合奥沙利铂方案治疗晚期胃癌的疗效观察

目的观察替加氟联合奥沙利铂方案治疗晚期胃癌的疗效和毒副作用。方法对52例初治或复治晚期胃癌患者采用替加氟1．0g静脉滴注,d1-d5;奥沙利铂130mg／m2静脉滴注4h,d1。28d为1周期。4个疗程后进行疗效评定。结果52例患者均可评价疗效及不良反应。总有效率为32．7％。初治者CR2例（6．7％）,PR9例（30％）,SD7例（23．3％）,PD12例（40％）,总有效率为36．7％,中位疾病进展期10个月,中位生存期18个月;复治者CR1例（4．5％）,PR5例（22．7％）,sD8例（36．4％）,PD8例（36．4％）,总有效率为27．2％,中位疾病进展期7个月,中位生存期12个月。不良反应主要是骨髓抑制白细胞减少、周围神经炎、均为Ⅰ～Ⅲ度,可耐受,无化疗相关死亡。结论替加氟联合奥沙利铂方案治疗晚期胃癌疗效肯定且毒副作用小,能完全耐受,值得临床推广使用。

Clinical observation of combined chemotherapy of tegafur and oxaliplatin for treatment of advanced gastric cancer

Objective To evaluate clinical efficacy and toxicity of chemotherapy with tegafur combined oxaliplatin （OXA ） in the treatment of advanced gastric cancer. Methods A total of 52 patients （ newly diagnosed or refractory disease） with ad- vanced gastric cancer were treated with tegafur 1. Og（ iv gtt Dayl - 5 ） and oxaliplatin 130mg/m2 （ iv gtt 4 hour Dayl }. 28 days was one cycle. After 4 treatment courses clinical efficacy was evaluated. Results The total response rate was 32. 7%. The pa- tients of newly diagnosed DR were 2 cases （6. 7% ） ,PR 9cases（30% ） ,SD 7 cases（23.3% ） ,PD 12 cases （40%）. The total response rate was 36.7%. The median time of progression was 10 months and the median overall survival time was 18 months. For the patients of refractory disease,CR was 1 case（4. 5% ） ,PR 5 cases （22. 7% ） ,SD 8 cases （36.4%）. The total response rate was 27.2%. The median time of progression was 7 months. The median overall survival time was 12 months. The toxicity was mainly bone marrow despression with leucopenia,peripheral sensory neuropathy,both being Ⅰ～Ⅲ in degree and tolera- ble. There was no chemotherapy related death. Conclusion Oxaliplatin combined with tegafur for the treatment of advanced gastric cancer has relatively high response rate and low toxicity. The treatment can be tolerable completely and is worthy of clin- ical promotion.
