CT与MRI在诊断早期股骨头坏死中的应用价值

目的：采用CT和MRI两种方法对股骨头坏死早期进行检查,观察两种测试结果的检出率,分析探讨其作用.材料与方法：选择我院2010年3月至2013年2月收治的76例患者（114例髋）确诊为股骨头坏死的患者,对其进行1、2、3、4、5期CT及MRI影像诊断,探讨其诊断准确率及临床表现.结果：检测1～2期敏感性CT为65.38％ （17/26）,MRI为88.46％（23/26）,差异有统计学意义（x2=3.9000,P＜0.05）.总体敏感性、单一囊变上两种检测比较差异无统计学意义（P＞0.05）.骨小梁结构模糊CT检出率为15.38％（4/26）,MRI检出率为73.08％（19/26）,差异有统计学意义（x2=17.5412,P＜0.01）.结论：在检查股骨头坏死的临床诊断中,MRI在股骨头坏死早期诊断率高于CT,在早期的细微的骨小梁结构模糊的诊断率方面,MRI显著高于CT,MRI用来诊断早期股骨头坏死有一定的临床应用价值.

The Application Value of CT and MRI in Early Diagnosis of Osteonecrosis of the Femoral Head

Purpose： To discuss the application value of CT and MRI in early diagnosis of osteonecrosis of the femoral head. Materials and Methods： A total of 76 cases ir our hospital from March 2010 to February 2012, who were confirmed as osteonecrosis of the femoral head, were diagnosed by 1, 2, 3, 4, 5phase CT and MRI. The diagnostic accuracy rate and clinical manifestations of CT and MRI were discussed. Results： 1 - 2 phase detection sensitivity of CT and MRI were 65.38% （17/26） , 88.34% （23/26）. There was statistically significant between two kinds of detection methods （X2 = 3. 9000, P 〈0. 05）. The differences of overall sensitivity and single cystic change were not statistically significant （ P 〉 0. 05）. The trabecular structure fuzzy detection rate of CT and MRI were 15.38% （4/26） , 73.08% （19/26）. There was statistically significant （X2 = 17. 5412, P 〈 0. 01 ）. Conclusion： In the clinical diagnosis of osteonecrosis of the femoral head, thediagnosis rate of MRI is higher than CT in eally. The rate of MRI in the diagnosis of early small bone trabecula fuzzy is significantly higher than that of CT. MRI has certain clinical value to be used for the diagnosis of early osteonecrosis of the femoral head.
