海南岛76例老年人上消化道出血的病因分析

本文就海南海岛７６例６０岁以上的上消化道出血的病人行回顾性病因分析。上消化道出血的病因中，消化性溃疡４４例，占５７．８％。胃癌９例，上１１．」８％，食道静脉曲张７例，沾９．２％，慢性胃炎６例，占７．８％，急性胃粘膜商变占５．２％，肝癌胃转移１例，占１．３％。病涸未明５例。在消化性溃疡中以胃溃疡最多。对于肝硬化并上消化道出血的病人除考虑食道静脉曲张破裂外，还要注意到消化性溃疡及门静脉高压性胃病引起的

ETIOLOGICAL ANALYSIS OF 76 ELDERLY CASES OF UPPER GASTROINTESTINAL BLEEDING IN HAINAN

The etiology of 76 cases of upper gastrointestinal bleeding(UGIB)in patients above 60 years of age in Hainan was reviewed in this study.Of all the causes of UGIB, peptic ulcer was noted as 55.8%(44 cases), carcinoma of the stomach as 11.8%(9 cases),esophageal varicosis as 9.2%(7cases),chronic gastritis as 7.8%(6 cases),acute gastritis as 5.2%(4 cases),and metastatic carcinoma from the liver as 1.3%(1 cases)respectively.5 cases were left unconfirmed in this study. Gastric ulcer was most common among all the peptic ulcers. It is mainly due to the degeneration of the gastric mucosa the pyloric sphincter which normally the reflux of the bile and intestinal juice. The high incidence of carcinoma of the stomach was attributable to the depressive immune surveillance in the process of aging and deactivation and mutation of genes inhibiting carcinoma in genes inhibiting carcinoma in cells. As for the UGIB patients with hepatic cirrhosis,esophageal varicosis as well as peptic ulcer and portal hypertension are to be considered at the same time.
