超声造影与增强CT在鉴别诊断肾实质性肿瘤中的对比研究

目的比较超声造影（CEUS）与增强CT（CECT）在鉴别诊断肾实质性肿瘤中的应用价值。方法选取我院于2008年4月30日~2010年4月30日期间门诊及住院收治的肾实质性肿瘤患者54例作为研究对象,分别进行CEUS与CECT检查,观察两种检查方法增强强度、增强均匀度、假包膜征、增强消退时间,比较其灵敏度、特异度、阳性预测值、阴性预测值、准确率。结果 CEUS明显增强42例,肿瘤可见假包膜征31例,呈快进慢出39例,与CECT相比差异有统计学意义（P〈0.05）。CEUS灵敏度（97.87%）、准确度（92.59%）显著高于CECT（80.85%、77.78%）,差异有统计学意义（P〈0.05）。结论 CEUS在肾实质性肿瘤增强强度、假包膜征、增强消退等方面与CECT有不同的影像学表现,CEUS对肾癌的诊断敏感度与准确度显著高于CECT。

Comparative study between contrast-enhanced ultrasound and contrast-enhanced CT in differential diagnosis of renal tumors

Objective To comparison the application value of contrast enhanced ultrasound（CEUS） and enhanced CT（CECT） in differential diagnosis of renal tumors.Methods 54 cases of patients with renal parenchymal tumor admitted in our hospital from April 30,2008 to April 30,2010 were selected,and all patients were given CEUS and CECT examination.The enhanced intensity,enhanced evenness,pseudocapsule sign,enhance extinction time of the two examination methods were observed,and the sensitivity,specificity,positive predictive value,negative predictive value,accuracy rate were compared.Results Compared with CECT,CEUS significant characteristics showed that 42 cases had significant intensity,31 cases had pseudocapsule signs and 39 cases had appeared enhancement pattern as ＂fast-in and slow-out＂,the differences were statistically significant（P 0.05）.The sensitivity（97.87%） and accuracy（92.59%） of CEUS were significantly higher than those of CECT（80.85%,77.78%）,the differences were statistically significant（P 0.05）.Conclusion CEUS has different imaging findings with CECT in aspects of renal tumor enhanced intensity,pseudocapsule sign and enhance extinction.The sensitivity and accuracy of CEUS for kidney cancer diagnosis are higher than those of CECT.
