室上速导管消融术后阵发性房颤发生的高危因素分析

目的分析经射频消融术治疗的阵发性室上性心动过速（PSVT）后发生心房颤动（PAF）的高危因素。方法收集2005年7月-2010年7月经RFCA的PSVT患者308例，回顾性分析发生PAF病例40例。评价指标包括年龄、手术时间、既往PAF病史等13项，采用χ^2检验和非条件Logistic回归多因素分析。结果40例患者均得到完整随访资料，单因素分析表明年龄、PAF病史、PsVT病史、其他合并症、左室射血分数、左房前后径、折返机制、房性早搏数目等8项是射频消融术后PAF发生的危险因素，多因素分析表明年龄、PAF病史、左房前后径为PAF发生的高危险因素，差异有统计学意义（P〈0．05）。结论RFCA治疗PSVT后发生PAF的高危因素包括年龄、PAF病史、左房前后径（P〈0．05）。PAF相关因素筛选有助于提高早期诊断率，及时处理，防止继发其他并发症。

Study of the high risk factors about complication of PAF during PSVT after RFCA

Objective To study the risk factors of compication of PAF during PSVT after RFCA. Methods 308 PSVT patients received RFCA were retrospectively collected from 2005 July to 2010 July,including 40 cases diagnosed of PAF, there were 13 evaluation indexes including of age,operation time, history of PAF, et al. All above were analyzed with Chi-aquare test for mono-factor and two dimensional Logistic regression analysis for multi-factor. Results d0 patients were follow-up,single factor analysis showed that age,history of PAF,history of PSVT and other complications,left ventricular ejection fraction,left front diameter,exhumation mechanism and room after premature beat the number were risk factors for PAF occurred after RFCA, multi-factor analysis showed that age, history of PAF, diameter of PAF occurred after left in the high risk factors,there is statistical significance （P 〈 0.05 ）. Conclusion Risk factors of PAF during PSVT after RFCA included age,history of PAF,diameter of PAF. Screen of the risk fators related to PAF will be usefull for early diagnosis of PAF and prevention of other complication.
