血清降钙素原在老年肺炎诊断及治疗中价值

目的探讨血清降钙素原在老年肺炎患者诊断及治疗中的价值。方法 120例老年肺炎患者为肺炎组,按CURB-65评分系统分为低危组38例、中危组45例、高危组37例,非感染性支气管疾病患者40例为对照组。在治疗前、治疗5d后、痊愈或死亡前检测血清降钙素原和C-反应蛋白水平,并进行比较分析。结果肺炎组治疗前血清降钙素原及C-反应蛋白水平均较对照组升高（P〈0.01）;治疗5d后低危组、中危组血清降钙素原水平明显下降（P〈0.05）,高危组治疗前、后血清降钙素原水平差异无统计学意义（P〉0.05）;治疗5d后肺炎组C-反应蛋白水平与治疗前比较差异无统计学意义（P〉0.05）。结论血清降钙素原对老年肺炎早期诊断、严重程度及预后判断有意义,敏感性优于C-反应蛋白。

Value of serum procalcitionin to the diagnosis and therapy of pneumonia in the elderly patients

Objective To evaluate the value of serum procalitionin to the diagnosis and therapy of pneumonia in the elderly patients.Methods A total of 120 elderly patients with pneumonia were divided into low-risk group（n=38）,medium-risk group（n=45） and high-risk group（n=37） according to CURB-65 criteria.And another 40 patients with non-infective bronchial disease were as controls（control group）.The serum procalitionin and C-reactive protein levels were detected and were compared before treatment,after treatment for 5 days,and when cured and died.Results Before treatment,serum procalitionin and C-reactive protein levels were higher in pneumonia patients than those in control group（P0.01）.After treatment for 5 days,serum procalitionin level decreased significantly in low-risk and medium-risk group（P0.05）,and had no significant difference in high-risk group before and after treatment（P0.05）.C-reactive protein level was not significant different before treatment from that after treatment for 5 days（P0.05）.Conclusion Serum procalitionin plays an important role in the diagnosis and prognosis assessment of pneumonia in the early stage in the elderly patients.The sensitivity of procalitionin is better than that of C-reactive protein.
