点阵式强脉冲光治疗黄褐斑和炎症后色素沉着的临床观察

摘    要:
目的观察和探讨点阵式强脉冲光治疗面部皮肤色素性疾病的临床疗效。方法应用点阵式强脉冲光对64例黄褐斑患者、65例炎症后色素沉着患者进行治疗。治疗波长为560~640 nm,脉宽为3~6 ms,每个光斑2~3脉冲,能量密度12~20 J/cm~2。4周治疗1次,3~5次为1个疗程。治疗前后采集和分析皮损图像,统计分析并做出评价。问卷调查和随访统计患者的不良反应、复发率和满意度。结果黄褐斑患者经点阵式强脉冲光治疗后,与治疗前比较,色素斑绝对分值与mMASI下降,差异有统计学意义(P均0.05)。治疗炎症后色素沉着的总有效率为93.85%。治疗总复发率为5.4%,烧灼、疼痛感等不良反应的发生率为14.7%,患者总满意度评价为92.7%。结论点阵式强脉冲光可用于治疗黄褐斑及炎症后色素沉着,改善皮肤外观。

Clinical efficacy of fractional intense pulsed light (FPL) in the treatment of melasma and postinflammatory hyperpigmentation
Abstract:
Objective To observe and discuss the effects of fractional intense pulsed light (FPL) in the treatment of melasma and postinflammatory pigmentation. Methods FPL was utilized for the treatment of melasma and postinflammatory pigmentation, with a filter of 560 nm, 2~3 pulses of 2~3 ms, energy density 12~20 J/cm2. The patients received treatment per four weeks, and one course included 3~5 treatments. Images of the patients collected before every treatment, and the data was analysed and evaluated. The badness reaction, the recurrence rate and satisfaction of the patients were added up. Results After treatment of FPL, absolute scores and mMASI score in the melasma patients decreased significantly. The efficacy rate in treating postinflammatory pigmentation was 93.85%. The overall recurrence rate was 5.4%, the incidence of adverse reactions such as burning or pain was 14.7%, and the total satisfaction rate was 92.7%. Conclusion FPL can be used in the treatment of melasma and postinflammatory pigmentation, which may improve skin appearance.
