45例儿童肾母细胞瘤临床分析

目的 探讨儿童肾母细胞瘤的治疗模式,提高其疗效.方法 回顾性分析45例儿童肾母细胞瘤患者的诊治过程,根据分期、组织分型、治疗方式进行分组,应用Kaplan-Meier法比较各组3年和5年总生存率(overallsurvival,OS)、2年无瘤生存率(disease free survival,DFS).结果 单纯手术组(n=7)3年和5年OS分别为100.0％和66.7％,与综合治疗组(n=36)的80.9％和72.8％比较差异均无统计学意义(均P＞0.05).综合治疗组2年DFS高于单纯手术组,差异具有统计学意义(61.9％ vs 17.9％,P＜0.05).不同分期和组织分型之间5年OS、2年DFS比较差异均无统计学意义(均P＞0.05).结论 综合治疗模式能提高肾母细胞瘤疗效.

Clinical analysis of 45 children with Wilms' tumor

Objective To analyze the therapeutic effect of Wilms' tumor treatments in children.Methods Fortyfive children with Wilms' tumor were grouped according to their disease stage,histology type and treatment method.Kaplan-Meier method was used to analyze the follow-up data,including 3-year overall survival (OS),5-year OS,and 2-year disease free survival (DFS).Results No significant differences were detected in 3-year and 5-year OS between patients treated with surgery alone(n =7) and those treated comprehensively (n =36) (100.0％ vs 80.9％,66.7％ vs 72.8％,respectively,P ＞ 0.05).However,2-year DFS in patients with comprehensive treatment was higher than that in patients with surgery alone (61.9％ vs 17.9％,P ＜ 0.05).There was no impact of disease stages and histology types on 5-year OS or 2-year DFS (P ＞ 0.05).Conclusion The standard multi-disciplinary treatment strategy can improve the therapeutic effect for children with Wilms' tumor.
