甲状腺未分化癌和甲状腺乳头状癌超声成像特征的对比研究

摘    要:目的结合疾病病理特点,探讨甲状腺未分化癌和甲状腺乳头状癌超声成像特征的差异。 方法选取2010年1月至2015年6月在本院接受治疗并经病理证实为甲状腺未分化癌的患者12例为观察组,另选取同期经病理证实为甲状腺乳头状癌的患者35例为对照组,观察两组患者临床特征和超声特征。 结果女性患者所占比例大,观察组患者病灶平均直径大于对照组,纵横比<1的比例高于对照组,且观察组病灶微钙化程度高、血流少不规则、超声回声更不均匀、边界更为模糊,以上差异均有统计学意义(P<0.05)。两组患者超声特征中病灶形态、囊性变、回声、被膜受侵的对比差异无统计学意义。 结论甲状腺癌患者以女性为主,未分化癌患者病灶纵横比一般都<1且具有微钙化程度高、血流少不规则、超声回声更不均匀、边界更为模糊的特点,值得临床参考。

Abstract:ObjectiveTo discuss the differences of ultrasound features between anaplastic thy-roid carcinoma and papillary thyroid carcinoma. MethodsPatients were selected from January 2010 to June 2015 in our hospital for treatment and twelve patients with pathologically confirmed anaplastic thy-roid cancer as the observation group, another 35 cases over the same period with pathologically confirmed papillary thyroid carcinoma were selected as the control group. Clinical features and ultrasound character-istics of the two groups were analyzed. ResultsWomen accounted for a large proportion of patients with thyroid cancer. Mean diameter of the disease was greater in anaplastic thyroid cancer patients, who were more likely to be lesions ratio <1, had higher micro calcification, less blood irregularity, more ultra-sound echo uniformity, and more blurred boundaries; all the above differences were statistically signifi-cant (P<0.05) . Characteristics of the two groups of patients with lesions in ultrasound morphology, vari-ants, echo, and contrast cystic capsule invasion were not statistically significant. ConclusionsThyroid cancer is female-dominated. Anaplastic thyroid cancer patients are generally with undifferentiated aspect ratios <1 and have a higher degree of microcalcifications, less irregular blood flow, more ultrasound echo uniformity, more fuzzy boundary characteristics, which is worthy of reference.
