卡维地洛对充血性心力衰竭患者QT离散度的影响

目的观察卡维地洛对慢性充血性心力衰竭(CHF)患者QT离散度(QT d)的影响。方法62例CHF患者随机分为2组,对照组:采用常规治疗方法(ACE 1/AR b、利尿剂、地戈辛);治疗组:在对照组治疗的基础上,加用卡维地洛治疗3个月。治疗前后同步记录12导联心电图,测量QT d及QT cd,同时测定左室射血分数(LVEF)。结果对照组患者治疗前后QT d、QT cd无显著变化(P>0.05),治疗组治疗前后QT d、QT cd有显著缩小(P<0.01);治疗组与对照组LVEF有明显差异(P<0.05)。结论卡维地洛可使CHF患者QT离散度缩小,可能是卡维地洛提高患者生存率的原因之一。

Effect of carvedilol on QT dispersion of congestive heart disease

Objective To discuss the effect of carvedilol on patients with congestive heart disease(CHF).Methods Sixty-two patients with CHF were divided into two groups: control group treated by conventional therapies.(ACEI/Arb,diuretic and digoxin);treatment group treated by carvedilol for 3 months in addition to conventional therapies.12-leads eletrocardiogram(ECG) was recorded at the beginning and end of the trial to measure QT disperse(QTd) and corrected QTd(QTcd).Meanwhile all patients were examined by echocardiography and left ventricular ejection fraction(LVEF) with echocardiography.Results QTd and QTcd in control group revealed no significant difference during the trial;whereas in treatment group the difference was significant(P<0.01).There was obvious difference in LVEF between the two groups at the end of the trial(P<0.05).Conclusion Carvedilol can decrease the QT dispersion of the patients with CHF,which may be the cause of carvedilol to improve the survival rate.
