体腔持续热灌注化疗在恶性胸腹水治疗中的应用

摘    要:目的评价体腔持续热灌注化疗在恶性胸腹水治疗中的应用疗效,探讨其临床适用性。方法选择2011年3月至2013年3月间收治的62例恶性肿瘤伴恶性胸腹水患者,随机分为观察组(32例)和对照组(30例)。对照组患者采用单纯腔内化疗,于腔内中心静脉置管抽液后注入化疗药物;观察组患者采用体腔持续热灌注化疗,于腔内置管抽液化疗联合体腔持续热灌注。观察两组患者的治疗有效率、Karnofsky评分及不良反应情况。结果两组患者治疗后症状均有所缓解,观察组患者的治疗有效率为93.8%,对照组患者为70.0%,两组比较差异有统计学意义(X2=5.98,P〈0.05)。Karnofsky评分改善情况比较,两组差异有统计学意义(X2=4.22,P〈0.05)。两组患者不良反应发生率比较,差异有统计学意义(X2=10.91,P〈0.05)。结论体腔持续热灌注化疗在恶性胸腹水治疗中有很好的疗效,适合临床长期推广应用。

Clinical study of hyperthermic perfusion chemotherapy for body cavity in the treatment of malignant pleural effusion and malignant celiac effusion

Abstract:Objective To study the hyperthermic perfusion chemotherapy for body cavity in the treatment of malignant pleural effusion and malignant celiac effusion. Methods 62 cases of malignant tumors with malignant pleural effusion were choosed in our hospital from March 2011 to March 2013, then they were randomly divided into observation group of 32 cases and a control group of 30 patients, the control group used intracavitary chemotherapy alone with the cavity chemotherapy drugs injected into the central ve- nous catheter aspiration, the observation group used the body cavity continuous perfusion, buih-in chamber the tube drainage chemotherapy commonwealth cavity continuous perfusion. Observed the efficiency of thera- py, Karnofsky score and adverse reactions. Results After treatment the symptoms were alleviated, the ex- perimental group of patients with a therapeutically effective rate of 93.8% , the treatment of patients in the control group only 70. 0% efficiency, efficiency comparing the treatment of patients, the difference was sta- tistically significant ( X^2 = 5.98, P 〈 0. 05 ) ; According to Karuofsky score, the two groups of patients com- pare efficiency, the difference was statistically significant ( X^2 = 4. 22, P 〈 0. 05 ) ; compare the two groups the incidence of adverse reactions, the difference was statisticallysignificance ( X^2 = 10. 91, P 〈 0.05 ). Conclusion The hyperthermic perfusion chemotherapy for body cavity in the treatment of malignant pleural effusion and celiac effusion has good efficacy.
