高频振荡通气治疗极低出生体重儿呼吸衰竭

目的探讨高频振荡通气(HFOV)治疗极低出生体重儿呼吸衰竭的疗效。方法对应用HFOV治疗极低出生体重儿呼吸衰竭21例的临床疗效进行总结分析。结果HFOV能明显地改善患儿的通换气功能,在HFOV治疗1/2小时、6小时、12小时、24小时、48小时后各项观测指标(包括FiO2、a/APaO2、PaCO2、OI)均有明显改善,治疗6小时PaCO2基本降至正常,OI和FiO2逐步降低,21例患儿中完全治愈17例,2例由于多器官功能衰竭、严重感染抢救无效死亡,2例因其他原因在48小时内放弃,无1例气漏发生,无1例慢性肺病(CLD)及晶体后纤维后增生(ROP)的发生。结论HFOV对极低出生体重儿急性呼吸衰竭的治疗有良好的疗效。
