原发性肺癌相关性肾病78例临床分析

目的分析原发性肺癌并发肾病的特点、诊断及治疗,探讨发病机制。方法分析我院收治的原发性肺癌675例中并发肾病78例（8.65%）患者的临床资料。结果78例患者中50.00%的患者单纯性蛋白尿,11.53%的患者单纯性血尿,8.97%的患者肾病综合征,24.35%的患者肾炎综合征,5.12%的患者梗阻性肾病,55.12%患者有不同程度的肾功能损害,大多数患者未针对肾脏病变采取治疗,67（85.8%）例好转出院,11（14.2%）例死亡。结论肺癌患者早期进行肾脏方面的检查对保护肺、肾功能有重要作用,肾损害可为肺癌诊断前的唯一表现,对于中老年人肾脏疾病患者,尤其是长期吸烟的男性患者,更应高度警惕肺癌的可能。

Analysis of 78 Cases with primary lung cancer complicated nephropathy

Objective To analyze the clinical characters of primary lung cancer complicated with nephropathy. Methods 78 cases primary lung cancer complicated nephropathy were from 675 patients with primary lung cancer in our hospital from 1998 to 2007. The clinical data were retrospectively analyzed. Results 50. 00% and 11. 53% of 78 patients manifested simple protelnuria and hematuria, respectively. 8.97 %, 24.35 % and 5.12 % were characteristic as nephrotic syndrome, nephritic syndrome and obstructive nephropathy, respectively. 67 （85.8 %） were improved, and 11 （14.2%） died. Conclusion It is important for lung cancer patients to detect the function of kidney. Renal damage may be the unique manifestation in early lung cancer.
