闭塞性细支气管炎伴机化性肺炎的临床分析

目的探讨闭塞性细支气管炎伴机化性肺炎（BOOP）的临床、影像学及相应的病理特点,提高对BOOP的认识。方法回顾性分析63例BOOP患者的临床资料,总结其临床、影像学、实验室及病理特点。结果咳嗽、活动后气短、咳痰、乏力、吸气末闻及爆裂音为BOOP的主要临床表现,常规实验室检查无特异性,肺功能呈限制性通气功能障碍,弥散功能降低,高分辨胸部CT及病理检查具有特异性,多数患者对糖皮质激素治疗反应良好。结论闭塞性细支气管炎伴机化性肺炎诊治困难,高分辨胸部CT和活体组织病理检查有助于提高闭塞性细支气管炎伴机化性肺炎的诊断。

Clinical and pathologic features of bronchiolitis obliterans with organizing pneumonia in patients

Objective To explore the clinical characteristics, chest CT and the pathologic features of bronchiolitis obliterans with organizing pneumonia（BOOP） in patients, and therefore to improve the recognition and diagnosis of BOOP in patients. Methods Clinical manifestations, chest CT,laboratory test and pathologic characteristics of sixty-three patients diagnosed with BOOP were retrospectively reviewed. Results The main clinical manifestations are cough, dyspnea, sputum production, malaise. The signs, symptoms and physical findings and laboratory test of BOOP in patients is nonspccifie. Pulmonary function test showed a restrictive ventilator defect and a impairment of diffusing capacity of carbon monoxide. The chest CT of hight-resolution, pathological and uhrastructural examinations can find specific features. Most of them were good effect to glucocorticoid treatment. Conclusions The diagnosis and treatment of BOOP in patients are difficult. The chest CT of hight-resolution and biopsy help to improve the diagnosis and treatment of BOOP in patients .
