儿童急性白血病血清IL—3的动态观察及临床意义

摘    要:
为研究血清IL— 3在急性白血病中的动态变化并探讨在白血病治疗过程中如何恰当选用IL— 3。采用酶联免疫吸附试验 (ELISA)方法测定 34例急性白血病患儿血清IL— 3水平 ,1 6例进行动态观察。结果 ,急性白血病患儿ANLL与ALL之间IL— 3水平无明显差异 (P >0 .0 5 )。完全缓解 (CR)后 ,血清IL— 3水平与治疗前比较 ,无显著性差异 (>0 .0 5 )。急性淋巴细胞白血病患儿治疗后高危型 (HR)组比标危型(SR)组血清IL— 3水平明显降低。经治疗CR后 ,有 1 0例IL— 3水平明显升高 ,6例明显降低 ;IL— 3变化与临床分型、获得CR时间及治疗效果有关。结果表明 ,IL— 3与白血病 (AL)治疗有关。建议 :在高危急淋患儿或完全缓解时间 >4周、骨髓衰竭、急性粒细胞白血病患儿应用IL— 3和化疗联合治疗。对缩短完全缓解时间及减少继发耐药 ,缩短骨髓抑制期时间 ,减少感染并发症可能起到重要作用。

KINETIC STUDY OF SERUM IL-3 LEVELS IN CHILDREN WITH ACUTE LEUKEMIA AND ITS CLINICAL SIGNIFICANCE
Abstract:
To study the successive change of the level of serum of IL-3 in children with AL and how to use IL-3 appropriately in treatment of leukemia. Solid phase sandwich enzyme linked-immunosorben assays (ELISA) was used to investigate the levels of serum IL-3 in 34 acute leukemia patient, 16 of them were detected successively. Results showed the levels of serum IL-3 were no significant difference between ANLL and ALL (p>0.05). No significant difference in the levels of serum IL-3 between patients of pre-treatment and patients with AL in remission (p>0.05). Serum IL-3 levels were significantly decreased in post-treatment with high risk patients than those with standard risk patients. In ten of sixteen AL patients with complete remission, serum IL-3 levels increased compared with that of pre-treatment, while in the other six decreased. It has shown that there was relationship between changes of serum IL-3 levels and clinical types, duration of complete remission, effect of treatment. Conclusion: IL-3 has a relationship with the treatment of leukemia, we suggest those who were ALL patients with high risk, delayed CR (above 4 weeks), bone marrow failure or AML should receive IL-3/chemtherapy combined treatment. It played an important role in shortening duration of receiving complete remission, reducing secondary drug-resistance, shortening duration of bone marrow suppression and reducing complication of infection.
