纤维乳管镜在乳腺导管占位病变诊断及治疗中的应用

目的探讨纤维乳管镜检在乳腺导管占位性病变诊断及治疗中的应用及意义。方法对2010年1月至2012年12月进行纤维乳管镜检查的158例乳腺导管内占位病变患者的临床资料进行回顾性分析。结果158例占位性病变纤维乳管镜检发现良性占位85例，癌前病变及恶性占位73例：乳腺导管癌29例，乳头状瘤病44例，乳头状瘤36例，炎性病变49例；纤维乳管镜组织学检查发现良性占位80例，恶性及癌前病变患者78例；纤维乳管镜检的敏感度及特异度分别为93．59％、94．12％。结论对于乳头溢液合并乳管占位的患者，纤维乳管镜可以明确病变原因及部位，并可对病变性质进行较为准确的诊断，有利于患者的及早治疗。

Study on application of mammary fibroptic ductoscopy in diagnosis and treatment of 158 patients with mammary duct space - occupying lesions.

Objective To explore the application and significance of mammary fibroptic ductoscopy in diagnosis and treatment of patients with mammary duct space - occupying lesions. Methods The clinical data of 158 patients with mammary duct space - occupying lesions undergo- ing mammary fibreptic ductoscopy in this hospital during January 2010 to December 2010 were retrospectively analyzed. Results Of these 158 ca- ses, benign lesions were diagnosed in 85 cases, precancerous or malignant lesions in 73 cases, breast ductal carcinoma in 29 cases, papillomatosis in 44 cases, papilloma in 36 cases and inflammatory lesions in 49 cases. Their histological examinations demonstrated that benign space - occup- ying lesions in 80 cases, and malignant or precancerous lesions in 78 cases. The sensitivity and specificity of mammary fibroptic ductoscopy were 93.59% and 94.12% respectively. Conclusion For patients with nipple discharge and mammary duct space - occupying lesions, fibroptic duc- toscopy can locate the position of lesions and make relatively accurate diagnosis, thus it may give the early and accurate diagnosis and treatment for these patients.
