断指再植术后再植指总体活动度恢复状况及其影响因素分析

目的探讨断指再植术后再植指总体活动度恢复状况及其影响因素分析。方法回顾性选择2018年1月—2020年1月医院收治的断指再植术后患者82例为研究对象,收集汇总患者的性别、年龄、文化程度、术后康复训练情况、缺血时间、术后心理状态、疼痛程度、断离性质、断指伤的原因、断离平面、医保情况、家庭年均收入、职业类型、术后皮肤温度、术后毛细血管反应、术后指腹弹性情况、合并基础疾病、受伤的手指类型等相关因素,进行统计学处理,确定并分析独立影响因素。结果断指再植术后患者的再植指外观恢复得分为15.59±3.45分,运动功能评分为14.27±4.13分,感觉恢复评分为16.38±5.39分,日常活动恢复评分为7.36±2.47分,恢复工作情况评分为6.78±2.59分,再植指功能总分为60.38±16.28分,其再植指功能活动恢复水平较差。单因素分析显示,年龄、术后康复训练、缺血时间、术后心理状态、断离性质、断指伤原因、断离平面、受伤的手指类型、术后指腹弹性、术后皮肤温度均为影响断指再植术后患者再植指功能活动恢复的相关因素(P<0.05)。多元线性回归分析显示,断离平面、术后康复训练、术后心理状况与断指伤原因均为断指再植术后患者再植指功能活动恢复的影响因素(P<0.05)。结论断指再植术患者术后再植指功能恢复与断离平面、断指伤的原因、术后心理状态与术后康复训练情况等密切相关,护理人员配合医师,采取针对性护理干预,促进其功能康复,对患者恢复生活能力、回归工作岗位并重返社会有着极其重要的意义。

Analysis of the influencing factors of the general restoration ability of the reproduction finger after the severed finger replantation

Objective To explore the influencing factors of the general restoration ability of the reproduction finger after the severed finger replantation.Methods Retrospectively select 82 replantation of severed finger patients admitted in the hospital from January 2018 to January 2020 as the study objects,the related factors such as gender,age,education level,postoperative rehabilitation training,ischemic time,postoperative psychological state,pain level,nature of amputation,cause of severed finger,plane of amputation,medical insurance,average annual household income,occupational type,postoperative skin temperature,postoperative capillary reaction,postoperative finger-abdominal elasticity,combined with underlying disease,type of injured finger,etc.The patients were collected and applied the statistical treatment to identify the independent influencing factors.Results The restoration appearance score of the replantation of severed finger patients was 15.59±3.45 point,the motor function score was 14.27±4.13 point,the sensory recovery score was 16.38±5.39 point,the daily activity recovery score was 7.36±2.47 point,the work recovery score was 6.78±2.59 point,the total score replant finger function was 60.38±16.28 point,the level of the replant finger function was poor.Univariate analysis showed that,age,postoperative rehabilitation training,ischemia time,postoperative psychological state,nature of severed finger,cause of severed finger,level of severed finger,type of injured finger,postoperative elasticity of fingertip,postoperative skin temperature were the related factors that effect the function of the replantation finger recovered after replantation(P<0.05).Multiple linear regression analysis showed that,level of severed finger,postoperative rehabilitation trainng,postoperative psychological state and the cause of amputated finger injury were the influencing factors of functional recovery of replantation finger after replantation(P<0.05).Conclusion The functional recovery of replantation finger patients after replantation was related with the causes of severed plane,cause of severed finger,psychological state after operation and postoperative rehabilitation trainning,ect.,nursing staffs should cooperate with the physicians,take the target nursing intervention,promote the pootients’function recovery which has a positive meaning for the patients regain the living ability,back to the work and society.
