纤维支气管镜在野战急救气管插管中的应用

目的:评价纤维支气管镜在野战急救困难气管插管中的应用价值。方法:60例需要气管插管进行救治的野外伤员,运用纤维支气管镜引导插管(A组)30例,直接喉镜(A组)30例,其中7例经直接喉镜操作2次失败改纤维支气管镜引导下操作,分析比较2组伤员气管插管操作所需时间、插管次数、插管成功率以及插管所致的即刻并发症。结果:2组伤员的一般情况比较差异无统计学意义。A组困难插管一次插管成功率显著高于B组(P<0.05),A组插管时间明显短于B组,而A组一次插管时间长于B组(P<0.05),纤维支气管镜引导下插管后即刻并发症显著低于直接喉镜(P<0.05)。结论:纤维支气管镜具有体积小巧、方便携带等优点;野战急救困难气管插管采用Olympus LF-GP型(便捷型)纤维支气管镜引导下插管成功率高,速度快,并发症少,是提高野战急救水平的有效方法之一。

Application of Fiberoptic Bronchoscopy in Field Emergency Tracheal Intubation

Objective To evaluate the value of fiberoptic bronchoscopy for the field emergency tracheal intubation. Methods Sixty patients underwent tracheal intubation in the field by fiberoptic bronchoscopy or laryngoscope. There were 30 ones by fiberoptic bronchoscopy assigned into group A, 30 ones by laryngoscope into group B. In group B, two patients turned to fiberoptic bronchoscopy after two times of failed laryngoscope operation. The operating time, intubation times, success rate and immediate complications of the two groups were compared and analyzed. ~ There was insignificant difference for the general health state of the patients in the two groups, but significant difference for the success rate of single intubation, intubation time and immediate complications. Group A behaved better than group B in the aspects of the success rate of single intubation, intubation time and immediate complications, but worse in the aspect of the time for single intubation. Conclugion OLYMPUS LF-GP-type（convenienee type） fiberoptic bronchoscopy can be used for field emergency treatment with high success rate and speed as well as rare complications.
