同步放化疗治疗局部中晚期宫颈癌的疗效分析

摘    要:
目的:探讨同步放化疗在中晚期宫颈癌治疗中的临床价值。方法将中晚期宫颈癌患者80例随机分为两组:单纯放疗组40例,行根治性放疗,体外放疗+192Ir腔内放疗。同步放化疗组40例,放疗方法与单纯放疗组相同,化疗方案为DDP 40 mg/m2,每周一次。结果单纯放疗组的有效率、3年生存率分别为75%、60%。同步放化疗组有效率、3年生存率分别为92.5%、77.5%。 P<0.05,差异有统计学意义。同步放化疗组骨髓抑制及消化道反应均较单纯放疗组严重,及时对症治疗后,患者均可耐受。远期直肠、膀胱并发症未见增加。结论同步放化疗能提高局控率和长期生存率,降低复发率,且并发症的发生率较低,有临床推广价值。

Curative effect analysis of concurrent radiotherapy and chemotherapy for metaphase and advanced ;cervical carcinoma

Abstract:
Objective To study the clinical value of concurrent radiotherapy and chemotherapy for metaphase and advanced cervical carcinoma .Methods Patients with advanced cervical cancer were randomly assigned to 2 groups:Radiotherapy group including 40 cases received radical radiotherapy , which was external radiotherapy combined with Ir 192 intracavitary radiotherapy .While concurrent radiotherapy and chemotherapy group including the other 40 cases combined DDP 40mg/m2 once a week.Results Response rate was 75%and 3 year survival rate was 60%for radiotherapy group , while 92.5% and 77.5% for concurrent radiotherapy and chemotherapy group.The difference is statistically significant ( P 〈0.05 ).Both the myelosuppression and gastrointestinal reaction were more serious in the concurrent radiotherapy and chemotherapy group which were well tolerated after symptomatic therapy .No increase of long-term complications of rectum and bladder was observed . Conclusions The concurrent radiotherapy and chemotherapy can improve both the local control rate and the long term survival rate and reduce the recurrence rate combined with low complication rate .It had considerable value to be promoted.
