电视腹腔镜下保守治疗输卵管妊娠效果分析

目的：分析腹腔镜下保守治疗输卵管妊娠的效果。方法：腹腔镜保守治疗输卵管妊娠42例，包括开窗术17例，成功率100％；胚胎挤出术或清除术7例，成功率100％；输卵管病灶注射MTXl5例，成功12例，成功率为80．0％；腹腔镜确认后肌注MIX75mg3例，成功率100％。总成功率为92．9％。结果：对32例未育妇女在术后3个月行子宫输卵管碘油造影(HSG)以确定患侧输卵管通畅度。开窗术、胚胎清除术、药物治疗术通畅率依次为75．0％，100％及76．9％。总通畅率为81．3％。结论：腹腔镜术输卵管妊娠的诊治，达到早期诊断和保守治疗的目的。轻柔的手术操作，术后的续后治疗可提高患侧输卵管的再通率。

RESULT ANALYSIS OF CONSERVATIVE TREATMENT FOR TUBAL PREGNANCY WITH VLDEO LAPAROSCOPE

Objective:To analyze the results of conservative treatment for tubal pregnancy with TV laparoscope.Methods:Forty-two cases of tubal pregnancy were treated by conservative treatment with video laparoscope .Among whic,17 cases were treated by fenestration,with a success rale of 100%.Seven cases by embryo extrusion or elimination,with a success rate of 100%. Fifteen cases by MTX injection in nidus(12 cases succeeded),with a success rate of 80%.Three cases by MTX injection in muscle after confirmed by laparoscopy,with a success rate of 100% .The total success rate was 92.9%.Results:In order to determine the rate of canalization of the suffering oviduct,32 cases of nonporous women were checked by HSG three months after operations.The rates of canalization treated by fenestration,embryo elimination and medication were 75.0%,100%and 76.9% respectively.The total rate of canalization was 81.3%.Conclusions:Diagnosis and therapy for tubal pregnancy with video laparoscope can achieve the purpose of early diagnosis and conservative treatment .Gentle operation and postoperative treatment ca promote the rate of recanalization of the suffering oviduct.
