股骨髋臼撞击症的X线表现分析

摘    要:目的 通过观察分析确诊为股骨髋臼撞击症的髋关节的X线表现,找出其影像特点.方法 对42例股骨髋臼撞击症的44个髋关节的X线平片进行分析、测量,并与50例对照组进行对比.结果 研究组44个髋关节和50例对照组在CE角、Sharp角、颈于角和头臼指数的对比中无明显差异,股骨髋臼撞击症分两型,杵型16个,臼型28个.股骨髋臼撞击症的x线征象包括股骨头颈交界处畸形、髋臼上缘增生、髋臼缘变尖和碎裂、深髋臼等.杵型和臼型的比较中发现CE角和颌十角有明显差异.结论 股骨头颈交界处的畸形和髋臼的异常形态是股骨髋臼撞击症诊断的主要征象,CE角和颈干角测晕数值异常、其他退行变的异常影像表现足诊断股骨髋臼撞击症的参考征象.

Abstract:Objective To analyze the characteristic appearances on X-ray plain film of hips with femoroacetabular impingement, to find roentgenodiagnostic features. Methods Forty-four hips with femoroacetabular impingement are analysed in imaging features, and compared with 50 hips of control group. Results Study group and control group show no significant difference in CE angle, Sharp angle, acetabular-head index and neck-shaft angle. Study group of 44 hips with femoroacetabular impingement is divided into cam type group (n=16) and pincer type group (n=28). The imaging features of X-ray involve femoral head-neck junction deformity, osteophyte in superior acetabulum, acetabular rim proliferations and sclerosis and coxa profunda, and so on. Conclusion The deformity of femoral head-neck junction and abnormality of acetabulum are the key points for diagnosis, the abnormal data of CE angle and neck-shaft angle, and the retrograde roentgenodiagnostic characteristic can only be a adminicle for diagnosis of FAI.
