| 摘 要： | 目的评价高容量血液滤过（High Volume Hemofiltration, HVHF）治疗重症肺炎的临床效果。方法对43例重症肺炎患者，均应用HVHF治疗，对比治疗前后中心静脉压（CVP）、平均动脉压（MAP）、氧合指数（PaO2/FiO2）、血乳酸水平、APACHEⅡ评分等指标的变化。结果HVHF治疗后，APACHEⅡ评分、血乳酸水平较治疗前下降，PaO2/FiO2、CVP和MAP水平较治疗前增高，差异具有统计学意义（P〈0．05）。结论HVHF治疗有利于血流动力学的稳定，可以改善氧合以及组织氧的利用，有益于重症肺炎的好转。 |

| Abstract: | Objective To evaluate the effect of High Volume Hemofiltration （HVHF） in the treatment of severe pneumonia. Methods 43 patients were treated with HVHF, central venous pressure （CVP）, mean arterial pressure （MAP）, oxygenation index （PaO2/FiO2）, blood lactate and score of acute physiology and chronic health evaluation II （APACHE Ⅱ ） were recorded before and after the HVHF. Results After HVHF, score of APACHE Ⅱ and blood lactate decreased while PaO2/FiO2, CVP and MAP rised significantly （P 〈0.05）. Conclusion HVHF is conducive to the stability of hemodynamics and can improve oxygenation and tissue oxygen utilization, which is effective to the improvement of severe pneumonia. |
